I came here after being in the hospital I was worried at first but then I met the nurse manager Sheri and my mind was at ease. She truly cares about her patients and spends time with you she even gives you and your loved ones her cellphone number if you need anything she is absolutely amazing I would come back here again if needed and send a family member here. Overall, my experience with West Melbourne Health and Rehab was quite poor. Not once in the two weeks my mom was there was I able to get her doctor on the phone, more often than not when I would try to call the line would be put on hold and ring for upwards of 30 minutes. When my mom was discharged, neither her physician or his assistant were notified and she was released with only 3 days of blood pressure stabilization medication. I was told she had enough for several weeks, and that it was all vitamins - not true. I asked to either schedule a phone call with her physician when she was being released or to see someone when I was there and I was denied, stating there 'isn't really much to know, she just needs to be reminded to use her walker and get her a shower chair'. I was given no instructions on her medication when I picked her up, a nurse stating to just 'follow the instructions' meanwhile she was sent with numerous medications, and again not all the right amounts. She had one medication that I had to take her blood pressure before giving to her, and I only found this out after calling 3 times, being told I would get a call back but never getting one. And 'there's no physician on the floor so there's nothing they can do'. I finally was able to get some assistance after 4 days of outreach, and I will be leaving a separate review for those individuals. Edit: it will not allow me to leave a positive review for Sherry and Leo separately, so I am adding it in here. Once they realized that my mom was discharged without proper medications, they were very helpful in making sure that she was able to get what we needed. Without them, my mom may have had to go to the ER as she is sometimes unable to maintain BP. I give them 5 stars all the way. I am not one to leave google reviews, but my moms health and safety is critically important and I don't recommend sending anyone you love here. My mom did say most of the nurses were very kind to her, but did have to file a report based on bad treatment she witnessed for her very old and not cognitively aware roommate.

Me: Sheri RN / C-wing unit manager 😊 His nurses: Debbie Jigi Scott Heidi Jason Bill His CNA’s: LaQuencia Sandy Marie Miranda Iberta I first wanna start off by saying thank you for taking care of my father the way you did he was there a month and at that time he looked good he was always clean. The facility was clean. He met some friends along the way. I would recommend this facility to anyone that’s needing to put a loved one in a rehab or for health reasons. He is part of you family now! Your going to miss him soooo much!!! If for some reason or any reason he needs to go back to a facility, you guys are the place. I will bring him. I highly recommend this facility to anyone searching for a caring and loving place. The staff was unbelievably friendly and was at my dad‘s every need. Any time I needed answers to questions whether it was case managers or administrator I got a call back in a timely manner and resolved my issues. Thank you again for that and closing. I am highly recommending this facility for your loved one. Thank you to everyone there in the staff for going above and beyond for my father. I mean that from my heart, you guys are the best.
I am leaving a review for Staff member Chrisford Smalling. Chris is a real asset to this facility and I feel blessed to know that he is there to help care for my mother when i cannot be there. The 5/5 is for him. He has taken extra time to get to know her and do little things for her that make her feel special. No matter how busy he is, he will make sure to explain things to her and make sure her needs are addressed. In a situation where residents often feel depressed, isolated, disrespected, forgotten, with a loss of independence and misunderstood with care needs not met … Chris adds that personal touch, quality of care and compassion that is needed for this vulnerable population. Nursing homes are plagued with understaffing, high costs and poor reimbursements which only adds to the burdens of their employees. Nursing staff, CNAs, MAs and all levels of care providers (including food services and maintenance) are rarely thanked for their tireless efforts in a very challenging situation. I am grateful for everyone there who is helping to care for my mother as I am unable to care for her myself. So, thank you and I commend management for having such a valuable employee as Chrisford Smalling.
