This is one of the saddest facilities anyone can put your Loved one in! It deserves 0 stars! The administer (Colinda) is very Rude, Unprofessional and Uncaring! The telephone system must be analog from 1992 because you can NEVER reach a Live Human and if you do, they DON'T know anything! Or, you get routed to the voicemail which is on a Ghost's desk because no one EVER returns your call! Colinda had the audacity to tell my Mother NOT to call her staff on their cellphones when that's the only communication you can get in or out of that place! My sister was placed here by Froedtert Hospital after she'd suffered a 2nd severe stroke which also brought on other complications including having a trachostomy, fluid removed from her brain and being paralyzed on her left side. However, after much Prayer and Excellent care from Froedtert medical professionals she was stabilized and cleared to go to Bria for further care and REHABILITATION which DID NOT happen! This place was a disaster from Day 1! They didn't even have food for her when she arrived, a staff member shared his lunch with her. A few of the staff members "pretended " to be caring and professional but, over the months that changed drastically and my sister suffered emotionally and physically. My Mother constantly complained to the Uncaring individuals who were in that unit and she was met with great unprofessional attitude and verbiage. She reach out to social workers through the State and voiced her concerns, they proceeded to look into them. I emailed and called the Wisconsin Quality care department with complaints. My sister became very sad and informed us that certain staff started to mistreat her verbally and handled her "roughly". She would call my Mother crying and begging to come home. Colinda or the social workers of Bria were NEVER available! On, November 17, 2022 my sister was sent to Froedtert ER because she was found "unresponsive" and was pronounced deceased at 10:32am! ONE OF THE WORST DAYS OF MY FAMILY 'S LIVES! We BELIEVE that due to the uncaring attitude, laziness and negligence of staff on duty that day my sister lost her life! BRIA OF TRINITY NEEDS TO BE SHUT DOWN! SO many deaths, complaints and violations and they're allowed to keep their license is a "slap in the face" to Families who've lost Loved ones! As of today, NO administer has even sent a sympathy card or condolence to My Family! I hope no other family has to face this tragedy! My Beautiful younger sister died 36 days before her 47th Birthday!

My mom was there for rehab, and I can say she was so pleased about her stay it was clean and they were very nice and caring and the social worker was very supportive and helpful with my mom though out her stay. I have nothing but positive feedback to give on them. Would recommend for anyone who wants their family member or loved one to be taken care of the right then this would be the place to send your loved one.
Majority of the staff are rude; lazy and lie consistently. They come to work just for the paycheck and care less about providing quality care to the patients. When you ask for help they make you feel bad for asking. You speak up, then the patient suffers later when family leaves. I could go on and on about our family’s experience there. We got our family member out of there as soon as we could. Just terrible experience and then helping with discharge was even worse and lies lies lies, avoiding talking to us and avoided helping with what they were suppose to do.
If you have a choice at all. Do not send anyone you love here. The absolute minimum of care in a dirty facility. Our loved one was left in a mess multiple nights when drains and connections weren't properly closed. They went unwashed for over a week. Oral care was never performed. I've witness a puddle in the common area once when I arrived. 3 hours later; I was leaving. The spill still there. A " caregiver" sitting near it watching TV in a wheelchair. The place was just gross! Workers were mostly seen outside on smoke breaks or sitting at the desk on their phones. The smell of human waste when walking in was ever present. It needs to be taken over by people who care.
If I could give this place a negative 10 stars I would! DO NOT go there or send a loved one there!!! The place is dirty, unsafe, everything is falling apart, holes in the linens, nurses yell at you, pain meds withheld for hours while suffering labor like pain from my surgery, food is inedible. I was tortured there when I went there for aftercare following spine surgery. several CNA's would not help me go to the bathroom when I was barely able to walk and a fall risk. The TOILET BROKE OFF THE WALL while I was sitting on it, causing me to fall to the floor on my knees and scream for help crawling to the door to escape dirty toilet water. I was transferred to another room that was filthy, broken toilet that didn't flush - when I asked for it to be plunged, a nurse came in yelling at me about it because no one told her it was broken, sink falling off the wall, I cut myself on the paper towel holder, the floor was filthy, doorknob to bathroom breaking off, tray falling apart. I was then told that the room had been deep cleaned! They did not tell any of the staff what happened, including my PT's. They did not look at my treatment or medication orders and often gave me meds at the wrong time. The nurses did not know how to pronounce the meds they gave me and at one point was told I was getting Narcan when it was Norco. PT made me do exercises I was not supposed to do causing more pain. They do not have ice packs or heat packs. I suffered damage to my knees from the toilet breaking and had to have xrays, a steroid shot and now risk future surgery to my knees because of them. I was nauseated and throwing up almost the whole time I was there because of the horrible quality of food and not getting food with my pain meds, I lost 5 lbs in 2.5 weeks because of not being able to eat the food and throwing up after a few bites. The dishes seemed dirty. I heard nurses screaming at patients throughout the night. 1 patient would yell "help, help, help" with no one responding. I was there for 2.5 weeks and only allowed 2 showers total. I did not get a sponge bath daily either and when I did, sometimes the towels would be dirty or have holes in them I didn't have sheets changed regularly and there were holes in them. They left my bandages on 1 week longer than they were supposed to and my skin blistered from the tape. I broke out in hives, became dehydrated and anemic while there. The nurses would not check my med schedule and would rely on what the previous shift nurse told them, which was often not true, so I wouldn't get my meds on time. I was supposed to have my vitals checked every 15 minutes after my fall, but the nurse came in after an hour and took blood pressure 4 x in a row, writing down different times on her sheet, saying she had to "catch up". They would constantly argue and gaslight me and make me cry daily. A male nurse yelled at me after I asked what med he had given me and said "why you takin somethin if you don't know what it is?" I said "because you're a nurse and I trust that you are giving me what I'm supposed to get", he responded with "you don't know me! you don't know what I can do to you!" I was terrified. I'm still having nightmares and in therapy because of the trauma I experienced there.
