We were extremely satisfied with Eli, the administrator, and his staff at Warren Barr Pavilion Lincoln Park. My wife, who has moderate dementia, received very attentive and understanding care as she recovered from an injury incurred in a fall. The nurses on her floor made sure she had things to keep her occupied, let her get some exercise, and treated her very nicely. They made all of the arrangements for my wife's appointments with MDs outside the building and made it very easy for me to visit with her. Eli and Patsy were very responsive to our needs and my wife's care.

My mom was in Warren Barr... March of 24. She was hesitant about going there.. However when she got there.. There was a care coordinator Theresa Palmer. She stepped in and made her feel right at home. She made sure her room was always clean. She made sure she had her coffee every morning.. Theresa encouraged the family to bring her frozen dinners and personal food for her refrigerator..because Mom had lost her appetite. She checked on her daily. Some of the nursing staff was friendly some were not. It gave us great comfort knowing Theresa was there with her. When Mom didn't want to do her therapy..Theresa went right up to her room and told her that wasn't an option. She also made sure she engaged in activities at the facility... Even if she had to bring the activities to her. Thank you Warren Barr and a special thanks to Theresa Palmer in aiding in Mom's recovery!
DO NOT bring anyone you care for to this facility. My mother was there 21 days and I brought her home. They have mice.the night shift turns off the phones to avoid calls,so you’ll NEVER get through. A nurse name Elaine Was giving my mother unauthorized melatonin to MAKE her sleep through the nite . The staff stole her lotion,hat,gloves and scarf.the have 3-4patients to a room. I’ve witnessed nurses cursing eachother out and literally fighting on the floor. They have a special needs activity aid watching the senior residents 🤦🏾♂️. He had an “episode” and cussed out the senior residents. I can go on and on. I wouldn’t allow my dog to die in this facility
This facility is amazing. The staff are so kind and truly dedicated to the patients. The Director of nursing and assistant director of nursing are so responsive to any questions or concerns and things get taken care of quickly. The social service director is excellent at providing support and discharge planning. Would highly recommend to anyone looking for rehab or long term care!
Theresa made us feel safe and comfortable when my granny was in her care. I can't express enough gratitude for Theresa's care for my granny. Theresa made us feel secure and relaxed when she arrived, creating a comfortable environment. The compassion and attention to detail were evident in every interaction, and my granny received exceptional care from Theresa. Knowing she was in such capable hands with Theresa was reassuring. Thank you, Theresa, for your dedication and kindness—you made a difficult time much easier for our family! Theresa is Highly recommend!
