This facility doesn't deserve any stars!!!! My mother in law was transferred from a hospital to this facility and we weren't there 5 minutes before we said "No way are we keeping our loved one here"!!! The place is so run down and dingy. The wall paper was peeling off the walls, the equipment was old and rusted, the bed side table was being held together by tape, the decor was from the 1960's and it looks like no one has updated anything since. Please don't send someone you love to this facility.

Colin Johnson is the very best. He came to hospital and talk to me and my mother bout my great aunt coming to stay there. He was there every step of the way while my aunt was there. I love Transitions. The nurses were the very best. Everyone there loved my great aunt. They took very good care of her. I would recommend Transitions to any and everyone and have recommended to any and everyone I've talked to. I really hope that when I'm old and frail they are still open because I would go there. My great aunts one nurse, Josh, was the very best. Its very clean too. I will definitely miss going there but I'm glad my great aunt is coming home and am so thankful so all the help they gave us with her.
My mother was at Transitions for about 3 years after a large stroke and was bed bound. She was well taken care of. She needed a lot of care. She never needed to go to the hospital until the very end. I was surprised they kept her going as long as they did. No bed sores. The staff was always very nice.
My mother is a long time resident and she loves it there. Her nurses are wonderful to her and they call me frequently with updates. Many of the staff have been there for years and that's always a good sign. They have tons 9f activities! They are always doing parties, bingo, bringing singers in, etc. It's honestly a very good place.
Now that my mother is out of there thank god, I have to speak up about this. This place is the worst most unprofessional facility I have ever seen in my entire life! Patients get verbally abused by the nurses. The nurses never want to help anyone. I found out my mom puked all over herself in the bed. Pushed the button and waited for an hour and no one came. She had to clean it all up herself! Apparently, the nurses don't care, but when family comes there to visit, then they put on an act and are checking on the rooms every 5 minutes. You can ask many different patients as well. I have asked a few myself. My mother was miserable there. Her last night there she pressed the nurse button. No one showed up until two hours later. I had to drive down there from Greensburg during my workday just so she could have a blanket! When I was there, I had to get a nurse when my mom needed her meds and wasn't feeling well. I truly hope and pray with all my heart that this facility gets sued and/or shut down one day! I wouldn't wish this place on my worst enemies. My mom was even terrified to say anything to the social worker as well because of how abusive the nurses are in fear of retaliation! All I wanted as for my mother to live her final months in piece. Well, it won't ever be at this place that's for sure. Want a cigarette? The nurses there have plenty to go along with their prison tats! Oh, and the blasting tv from some of the other patients there as well all night so my mother could never even sleep. If this were Compton or the damn hood, this place would fit right in!
