I came to Trinity because I broke my foot and my fibula by my ankle when I blacked out at home because of four serious illnesses that I didn’t even realize I had. And then I acquired a fifth illness after my staying in a different facility that was worse than the first four. I came here from a second stay in the hospital and I requested Trinity because I’ve known people that had been here before as well as some that are here now. I’m so thankful that they accepted me in this facility because just after 11 days here I have actually felt really good again and I was able to go through my rehabilitation because of the team that’s here at Trinity. This team has exceeded my expectations starting with the Admissions Team, Chaplains, Doctors, Nurses, STNA’s, aids, Rehab Team (personal trainers), Great food team, Comfort Team, Cheerful Activities Team (Including a Ladies Tea Party) an awesome Cleaning Crew plus so many more that helped me to have an attitude to do my best and they have been there to cheer, coach, and support me to want to work harder to exceed my expectations on my plan to go home again. My expectations of coming to yet another facility were all met plus so much more! Thank each and everyone of you for having the absolute best work ethics to cover all aspects of a persons needs to heal and be made whole again in an amazing environment! My daughter said to me “You sure you don’t want to move in?! Lol” after my sharing all the events as they happened including pictures of the room, amazing food and all the wonderful care I have been receiving and so much more! This could be my second home, because I do want to go home soon. If I would ever have to go through rehabilitation again my only choice would be Trinity of Beavercreek and, yes, I would be happy to wait for a bed to open.

My father has been at Trinity of Beavercreek for a week now after a hospital stay. I have been very happy with the care he’s receiving and he is also happy with it. we are in the fast track rehab program so I can’t comment on the other types of care at the facility, but the rehabilitation fast track rehab has been wonderful. The nurses go out of their way to help and I know they are so overworked, but they are so good and so kind. And the food is really good! My dad loves it.
My Dad was at Trinity Rehab after being hospitalized, it’s called Fast Track rehab but I’d hate to see slow track. To set the record I don’t always send bad google reviews I also praise business’s when they are good as well. I also try not to complain unless I have an idea for a solution or suggestion to make it better. I am a retired nurse and know first hand what it is like to work short staffed. this place needs to educate nurse and hire accountable people with integrity that which they do not have at this time. Trinity is SHORT staffed! I don’t know how anyone comes out alive. Most of staff is nice but they don’t follow through with promises. And call light waits are NOT SAFE. My Dad on average waited 20-30 min but a couple times it was over an hour!! Not acceptable!!! My Dad had a suprapubic catheter which comes out of your belly from your bladder, due to prostate cancer (too much radiation-caused this) this is not that common so no one knew how to irrigate it or replace it, and The staff tried to replace it and Dad said it was traumatic as they put in a tube took it out put another one in etc. then he had blood in his urine for days. He also started having pain in belly I had to ask many times for someone to irrigate it and it took along time because they had to find someone who knew how to do it!! As management of this place when you have a patient with a new or different problem that no one is educated on -YOU NEED TO EDUCATE THE STAFF! this was never done. If you ask them to order something it took days to get anything, with constant reminders from the family. Dad had ants in his room they sprayed once, it did not help, we asked again and they said they put in a work order, Dad had to keep his feet up on bedside tray so ants wouldn’t crawl on his feet! No PT on weekends, usually, maybe once or twice while he was there. Daryl was good and he was a good cheerleader- they need more Daryls. Julie the speech therapist was an angel she was such a kind and thoughtful person and gave Dad good tips on how to eat without choking. Mostly LPN’s run the place which is fine I’ve worked with LPN’s smarter then RN’s but there was one male nurse, BSN mind you, who was suppose to get a urine specimen, to see if Dad had a UTI he came in and started to take apart Dads catheter and I said wait what are you doing- you need a needle to access the port, he just kept on going and stuck the dirty Catheter down into the specimen bottle and got some urine and went out of room. I told his LPN preceptor what he did and she goes “HE told me he knew how to do it!” so she came in and did it right with a needle!! Educate your STAFF!!!!! Communication is awful and you have to constantly remind everyone what you need or want. Brooke the head nurse was great face to face and seemed to care, but things were not done as she said they would be. Upon discharge we were not told that we had to set up transport. We were waiting for a transport and found out— nope- we have to do that? what?? then NO ONE walked us out to help get him in our car???? what in the world!! Dad had oxygen and aids were not suppose to mess with it, but they did, and they did it wrong! I came in and Dad said he dreamed he was suffocating- well he was.... they had hooked up his oxygen wrong and he did not get any ALL NIGHT!! So I put tags on each tube explaining how to do it because I knew aides were going to be messing with it. There is absolutely NO CRITICAL thinking at this place!! Upon discharge Dad must have had a UTI- from that urine that was sent, but we were never told that and I have no idea if it was reported to Courtyard of Centerville where he was going to live. But He was ok for the first few days at Courtyard of Centerville- no symptoms, then he got Urospesis and died April 13th, 2025! Trinity Rehab you are so negligent!! Please do not send loved ones here!! I truly feel Dad would still be alive if we had not sent him to Trinity he was there over a month and it was horrible. There was more but it won’t allow more text!
Our Dad has been in the fast-track rehab facility at Trinity Beavercreek for about 7 weeks. He is due to be discharged very soon. The staff has been professional and attentive to Dad’s various needs and our many requests. The communication via email, phone calls and meetings has been super beneficial. The doctors, nurses, aides and therapists have treated Dad with dignity and compassion. They are always willing to update us on his progress and status. The facility is clean and well kept. Overall, we are pleased with our experience and would highly recommend Trinity Beavercreek.
My wife was at your great facility, Fast Track In-Patient Rehabilitation, United Church Homes Trinity Community at Beavercreek, for five weeks. The care that she received in all aspects of her treatment was exceptional with the staff providing excellent PT, OT and nursing care throughout her very productive and successful stay. She was able to recently return home due in large part to the care and treatment provided by the Trinity staff. Of special note are the following staff members: Dani - PT, Josh - PT, Daryl - OT, Cassie - STNA, Brooke - Discharge, and Erica - Admissions. Everything at Trinity rated five stars, with the exception of the food service (quality). For anyone that needs In-Patient PT Rehab, choose Trinity at Beavercreek.
