I can't say enough wonderful things about this facility. The staff is so kind and patient with my mom. Like any facility, they have turn over of staff, but they weed out quickly who needs to go. I love my Mom's aides Donna(we miss you), Amy, and everyone else. Her stay has transitioned, and we are sad to leave such a wonderful facility but keeping Mom in the Autumn Lake brand. Thank you for taking such great care of my Mom.

I don't do many reviews but my mother-in-law couldn't stop raving about her nurse on the day shift. Ebony was a doll and extremely professional. The food is fair, and the intake was a bit cold. However, the overall experience was stellar as she felt there was an attentiveness that was above and beyond which was far more important than having high end food.
Finding a facility you can trust isn't easy, but our experience at the Subacute exceeded expectations. A family member just had a short stay here and we were very happy with the PT care he received. The rest of the staff was consistently helpful and attentive, especially Bre in hospitality, who went above and beyond to ensure we had everything we needed.
I wish I could give this facility zero stars. Any question you ask staff, expect to get attitude and resistance. They will tell you that “protocol” is to rotate patients every 2 hours. I can assure you, if you sit with your family member, you will never witness this happening. When issues were addressed the only response received from management employees was that “staff needs additional training”. With that said, if you don’t mind leaving your family member with unqualified nursing staff, this shouldn’t be an issue for you. If your family members needs to be changed for any reason, don’t expect AL to do this in a timely fashion. Management will say that staff is required to look in on your loved one every 2 hours. Again, sit here long enough and witness the lack of attention your loved one will received. Don’t expect calls back. Even when you have documented the amount of times you have tried to reach someone for information, the extensive excuse list will be utilized in this situation. The case worker will not return your calls unless they seem “urgent” so make sure you sound extremely dramatic when you call so that they take you seriously. If you try calling the nursing desk for any update on your loved one, or to simply say hello to them, you’d have a better turnaround time sending a letter by usps. Staff does not respond to calls, emails or messages left at nursing station. You only receive a response if you elevate your concerns all the way up. Hallways have consistently smelled like urine at every visit. Staff throw supplies on the floor rather then throwing them in available trash cans. This review is based on actual experience and pictures/videos have been taken on numerous occasions validating my response/review. Bed sheets that were dirty on a Saturday we’re still dirty on a Monday. Absolutely do not send your loved one here if you expect the facility to be clean and organized. The aides here provided better service than actual nursing staff. If you also expect compassion for your loved one, look elsewhere. Staff seemed extremely annoyed if your loved one needed anything. Staff goes to work for more of a social hour than an actual honest care for nursing. Please avoid this facility of your insurance covers this place. An actual patient there stated that staff only asks family members if they need anything to be “smoke and mirrors” but they never actually ask the patients what they need or if they need any help. Avoid!!
8 months ago I gave a good review. But in 8 months things have definitely changed!! Our family experience the most terrible care imaginable. My father had been there for three weeks for therapy while there he developed a terrible cough and was having difficulty breathing. They neglected to have him tested for Flu or RSV which apparently was running rampant. As his condition rapidly deteriorated they refused to call 911 and we’re going to leave him overnight to die. We thankfully called 911 where he was admitted with Flu, RSV and pneumonia and numerous severe bedsores were found. There is no excuse for this type of neglect!
