The Springs at Monarch Landing is a wonderful 5-star facility. My mom recently was at the community for short-term rehab. Everyone was exceptional (from all departments) and provided excellent care during her stay and I just want to express my thanks to everyone for their kindness.

The Springs at Monarch Landing have exceeded my expectations and the customer service was outstanding. I highly recommend this facility. This is my second time here and they have not disappoint. The team is always friendly and helpful. The staff is professional, I am glad I chose The Springs.
We moved to the independent living apartment this year in March. The past 9 months have been enjoyable. We met many mind like people here and established good friendships. The management and the staff are very helpful and listen to the residents need. The adjustment has been easy, We are very happy with our decision. I am 74 years old, my wife and I are looking forward to making our retirement life here enjoyable and meaningful.

This does not mean best practices were followed or that serious concerns should be dismissed. Additionally, state charges related to negligence are currently under review, and federal scrutiny of discharge practices is expanding. There will be an IDPH hearing addressing this at end of month, 🚨 Transparency means addressing questions directly, not deferring to prior reviews. But Since you referenced IDPH’s oversight, I would like to highlight a public record from the Illinois Department of Public Health (IDPH)—Case #NH-20-C0045 (December 2019), which specifically cited The Springs at Monarch Landing for: • Failure to provide adequate supervision of non-ambulatory residents. • Staffing deficiencies that resulted in patient neglect. • Patient care failures that put residents at risk. 🚨 This raises the question: How have these issues been addressed since then, and what assurances can you provide that similar failures did not contribute to Joanne’s case 2. Unanswered Questions About Joanne’s Care Remain If The Springs at Monarch Landing is fully transparent, I would appreciate direct responses to the following: • Why was Joanne—a high-risk, oxygen-dependent patient—left for extended periods without proper monitoring? • Staff records confirm the last nurse check was at 12:30 AM and a CNA check at 3:00 AM, yet she was not found until 5:15 AM by an outside technician. • How do you explain the missing oxygen tubing? • No official response has been given regarding why her oxygen tubing was not in place when she was found unresponsive. • Why was her gown removed and placed on a chair? • If staff previously stated that Joanne could not get out of bed on her own, how do you reconcile that with the scene in which her gown was found elsewhere? • Why was Joanne discharged to SAR when medical records reflect palliative care needs? • If transparency is a priority, I request documentation of the discharge planning discussions that justified continued SAR placement instead of hospice. 🚨 A true commitment to transparency means directly answering these critical questions. 3. Moving Forward: Ensuring Accountability & Clarity My goal is to ensure an accurate and complete understanding of what happened and to prevent similar cases from occurring. If The Springs at Monarch Landing truly exceeds all regulatory standards, then addressing these outstanding concerns should not be difficult. The springs at Monarch Landing is one of the best places for skilled nursing care and rehab with caring and loving staff. Highly recommendable.
