I’m a huge fan and customer. Both my mother and step-mother have been in this facility. My step-mother is still there. Both of these parents were coming off of hip surgery and both received excellent physical therapy at the facility. Both received kosher food and were included in holiday services when they wanted. My step-mother needs extra care and she receives it constantly. We can’t say enough about the staff, ownership and leadership—all of whom are involved in the care of their patients. The Five Towns Premier Nursing and Rehabilitation Center has truly lived up to its name. My father is currently a resident there, and our family couldn’t be more grateful for the care and environment he’s receiving. The facility is always spotless — clean, bright, and inviting. The rooms are well-kept and feel new, which makes a huge difference in comfort and peace of mind. The staff are consistently kind, attentive, and genuinely caring — from the nurses to the aides to the administrative team. You can tell this is more than just a job to them. The rehab department is excellent — modern, professional, and effective. We’ve already seen meaningful progress, and they’ve kept us informed and involved every step of the way. And the location can’t be beat — right in the heart of the Five Towns, easy for family and friends to visit. We feel confident knowing my father is in good hands. Highly recommend Five Towns Premier to anyone looking for top-quality care in a warm, respectful setting.
Out of all the long-term and short-term care facilities I’ve visited while searching for the best environment for my loved one, this one truly stands out. From the warm and welcoming atmosphere the moment you walk in to the incredible personalities of the staff, every aspect of this facility feels thoughtfully designed. The cleanliness and the hotel-like environment—far from the typical hospital setting—bring me great comfort, both when I’m there and when I’m away, knowing my grandmother is safe and well-cared for. I’d like to specifically recognize Kim from the second-floor respiratory unit. Her genuine personal interest in my grandmother’s condition, her attentiveness to what happened, and the detailed care plan she shared about how she and her team would support my grandmother were major factors in our decision to choose this facility. It’s clear that she is deeply committed to her patients’ well-being, and that level of care means everything to our family. I’m excited to continue working with this facility as they help my grandmother regain her lively, feisty spirit so she can return home to her family and friends.
No one ever wishes that they need a place like this but when you do you can sleep better at night when the care provided to your loved one is as compassionate, attentive, and detail oriented as it is at Five Towns Premier. My grandfather needed a short term rehabilitation center after a hip fracture. After much deliberation and speaking to many centers we chose this one for proximity to our homes and the Russian program that is run by Natasha. We are so glad we did! Sadly, my grandfather's dementia has progressed and we have transitioned him from short-term rehab to long term care here but he is in such great hands! Natasha, as well as the rest of the staff, make sure he is safe, happy, and cared for as if part of their own family. They routinely have fun events for all the patients, specific Russian gatherings and tea parties regularly. If my grandfather is not in the mood to attend or partake Natasha personally visits him to ensure he is ok and brings some of the festivities to him. The PT department has helped with his physical recovery tremendously and he is able to once again do some basic activities of daily living independently. All of the nurses ensure he is clean and well taken care of. A difficult situation for any family to be facing, but we find peace in seeing how well he has been cared for here for nearly a year now.
This was my first time going into the facility. It was clean and an warm atmosphere. Furniture was up to par. I will update review in a month, as my relative just arrived
