My mother was in a bad situation at another skilled nursing home after falling and breaking her arm. We were able to move her with the HUGE help from Director of Admissions Jessica Butker. She and the staff at Southwestern have been amazing to my mother and are helping her get back to her old self. I can’t say enough about her PT team led by Patty and nurses and aids. Matt has been so kind to my mom. Thank goodness there are people in this world who care about the job they do for others as it shows everyday. If you have a loved one in need of short or long term care I would highly recommend Southwestern Nursing and Rehabilitation Center.

My very good friend is a resident here and I came to visit her. She was on hospice and Tracy the hospice nurse was there. Very sweet and attentive to her needs. As my visit continued, her nurse and several aides addressed her needs. They were all very kind to her! The nurse administrator, Gia, has the kindest heart. Impressed with her patient care! Thank you!
The nurses and staff have mostly been nice and dona good job. However, when you walk in the lobby and come to the building looks beautiful, when you get on the units they are not as beautiful. Not clean public restrooms is in great need of repair. The rooms are ok also need updated. No chairs for family to sit due to small size of rooms. There is a large hand written note on closet door not to take clothes to laundry family will clean but they took his clothes to laundry anyway hasn't got them back. When you call the phone rings too many times before answering sometimes they are nice sometimes not so much The care and therapy they do is excellent my husband likes the food. The therapy staff are very good and welcoming to family and nice to the patients.
Nursing staff take great care of my Mum. Checked in on her after she has a breathing issue. Called me after the episode. Always keeps me updated, either by phone or when I visit.
Staff very pleasant and welcoming. Willing to work around visitor's schedule when considering pt rehab needs. Room and pt very clean.
