The elderly shouldn't be going to bed hungry and that happens often. The kitchen, especially at night and on weekends get zero stars. Tonight's food came up as though it was straight out of the refrigerator and put on a plate. The pasta was crunchy and inedible. I buy a lot of my own food so I can eat but I'm quite sick and have no energy. The nurses and LNA's are top notch and overworked and understaffed. Donna, the receptionist has helped me several times when I was having a difficult time. She's a definite asset to this company. It's time for me to start looking for somewhere new. I have no clue how they got four stars, that's a joke. I'm probably going to be evicted for this review. Don't bring your parents or family members here.

The care that my mom is given is top notch. The nursing/care staff are wonderful. The food is horrible and is insulting the residents that they would even give it to them. It is the only complaint I have at the facility. The person that stands out as a superstar is the receptionist Donna! Every week without fail I am greeted in the kindest way. But Donna is not just a receptionist. She is a friend to the residents and always takes time to have little conversations with them. I know thst my mom and her small group of friends refer to Donna as a friend, and I love that. Today I was visiting while a resident was extremely agitated and wanted to leave the facility in his wheelchair. It got to the point he was trying to strike the worker who was trying to calm him. Donna stepped in and tried to intervene in a kind but firm manner. When it didn’t work she went to get the worker additional help and quickly the situation was rectified. Donna is amazing, kind and obviously will step in when needed. I am glad that my mother has people like Donna around her. It makes having to leave my mom in a nursing home a little easier.
If you love your Parents and your Grandparents, Do not send any of your family to premier rehab.There inability to care for there patients and bring your loved ones to their Doctor appointments, surgical appointment etc is horrible. I give this place a zero.
Absolutely terrible rehab. My grandfather was sent here. The food is inedible when you receive any food. Sometimes they forget to feed him and if it wasn’t for family visiting he wouldn’t even get food some days. My grandfather was sent there for rehab to regain strength and they have done nothing to help him regain his strength. They won’t use bed or chair alarms and claim that falls are unpreventable. My grandfather has had multiple unsupervised falls while he’s trying to get himself to the bathroom because no one responds to the call bell and there is no bed alarm to alert them he’s getting up alone. He is on a blood thinner and they do not send him to the hospital to be evaluated for a brain bleed or other injuries after the falls even though no one knows if he hits his head or loses consciousness since the falls are unwitnessed. My grandfather now can’t walk and needs a lift to move him because they have let him fall and be injured repeatedly.
Amy the nurse who is in charge of transportation is wonderful. Amy goes out of way to coordinate patient's doctor appointments and to make sure a driver is available even if she has to drive.. She also pitches in when needed. Anne and Jenn in activities are wonderful! I enjoy the activities such as my bingo and trivia . They make sure to invite all residents ifd bring Gus to
