This facility is excellent whether it's for rehab or nursing/hospice. They have plenty of activities, spacious private rooms & great meal options. But most importantly is the kind & compassionate staff. Every time they came in my mom's room, her face lit up. They went the extra mile to make her feel special & loved. I can't thank everyone enough for the wonderful care you gave my mom, Lorraine Kurszewski. ❤️

If you want an amazing nursing facility for your loved one Portage County Health Care Center (PCHCC) is amongst the best! As a healthcare provider that has worked at over 10 skilled nursing facilities, PCHCC has been the most patient centered facility that I have worked at. In a healthcare world that has had increased focus on profit and reimbursement, PCHCC has continued to demonstrate a patient centered approach.
The staff at Portage Couny home are a group of very caring indivualsl, who together, do an outstanding job. My Aunt is here and we were able to come in and do some activities with here that made her day. It involved doing some tasks that would have been very easy for staff to say, that can't happen and leave it at that. However their enthusiasm for our project was heartfelt and appreciated by family and by my Aunt more, than they will ever know. Thank you all for making it happen, and allowing us to make a woman's, not only day but her YEAR!!!!!!!
I was extremely unhappy with my mother's care here. They broke her toe taking her on the lift to the shower. Unbelievable. So many other issues.
Amazing staff! Amazing place to be!!! Truly have a passion for the care they do and the lives they touch.
