I just want to give this facility a shout out for all the care they have been providing to my grandma. My grandma is a very complicated patient who suffers from dementia and some underlying psych issues that cause behaviors. This facility has researched to try to find new ways on being able to meet her needs to keep her and the other patients safe. As I have worked in the LTC community I’m very familiar with rules, regulations and quality of care for patients. I can say this does not happen everywhere as the last facility she was at just shipped her to the ER and refused to take her back. Leaving her “homeless” and in the hospital for 22 days waiting placement. This facility accepted my grandma knowing she might be difficult, but have given her a chance. I just want to send a thank you to all the nurses, CNAs, dietary- just their entire team for their support. Your work does not go unnoticed from me and my family. And Misty, (their DON) thank you personally for your dedication and passion to your patients and team.

Understaffed, under trained staff and dirty. My mother was hospitalized because of the lack of care she recieved while here (the hospital Hotlined the facility). Memory care side was often left unlocked - my mother got out and made it to another nearby business before she was discovered. (The facility had to hotline themselves over this one). Floors never cleaned in my mom's room. Same stain on floor for months. Often found my mom sleeping in other rooms and other residents in her room. Do not recommend!
My mom was put there and this facility was filthy. Floors never mopped equipment was dirty. The social worker was awful no warmth or caring in her! Stone cold liar. If you talk to them they always have two people so one person can lie to you and the other will swear to it! My mother’s belongings turned up missing. Then we were accused of taking it. Does that make any sense. I would not put my dog in this place. They were not happy because the person that put her their did so knowing that the bill would never be paid. Terrible place for $5000 a month! Do not put your love one in this place!
My mother-in-law was in ONCC for almost 4 yrs. She recently passed away at age 91. From the get go she had excellent care even when dementia became worse. They called us often to see if she had an item that she insisted was lost. In all cases she was confused and we had the missing items. They were patient at times I'm sure it was difficult to be patient. Once she called us on the phone to say she needed help. She had fallen out of bed. Workers came in not knowing I could hear all conversations. They were so sweet and kind lifting her up and joked that she needed to pick a better place to sit down in the future. They kept us informed during covid. Never entered that I didn't see smiling faces.
I put my mom here for 5-day respite care through Hospice. Her experience was horrible. She they forced her to take meds she did not want and one of the nurse's told her that I told them to crunch them up and put them in her food if necessary but make sure she takes them. That is a lie. She was feeling anxious the day she went so the nurse ordered a low dose of Xanax for her to take if she wanted it. They included in her meds and lied about what they were and ended up giving her a double dose of it the first night. She was so out of it the next day that she doesn't even remember having a bowel accident in her pants. I was going through her dirty clothes when we go home and they didn't even bag the pants separately or rinse them out. There were more Xanax missing than any other meds. She was supposed to get Restasis eye drops twice a day. They put them in once during her entire stay. The Restasis and Zofran were missing from her meds when we got home so I called the facility. They can't find them. I am waiting for a call back from Mindy, D.O. of Nursing, to see what will be done about that. My mom will never see the inside of this facility again.
