THIS PLACE UNALIVED MY SISTER! MY SISTER WAS ADMITTED ON 2/6/2025 AFTER HAVING 6 PLATES AND 13 PINS IN HER ARM. SHE DIED IN THE CARE OF THIS FACILITY ON 2/8/2025. I received a phone call at 5:48am Saturday 2/8/2025 informing me that my sister died. No I'm sorry for the news, sorry to tell just zero sympathy, are you her sister, she died. They then called me back at 5:55am asking where I wanted to send my sister's body. I can't sue for negligence due to the Florida Free Kill Law and my sister being unmarried and no children. DO NOT SEND YOUR LOVED ONES HERE THEY WILL LET THEM DIE. I have called 15 times since Monday trying to speak with the D. O. N. Because I still don't know my sisters cause of death and these people refuse to tell me or return my calls.

My brother has been treated like royalty they accepted him with all his ailments. The entire staff is just wonderful. The facility is old but very clean
I've been here for a month and the staff here has been absolutely amazing. From my very first day till now I've had great care. My CNA's Alyssa who is so helpful and sweet, Brenda aka(Grandma) who is amazing,Katrina who went above and beyond to help me, Remy a great young man, Alexis, and last but not least Tammy who is awesome also. The nursing staff Lila , Shauna, Dawn, Pam and others who's names I can't recall have all been wonderful caring and compassionate. The therapy crew here including Shanteria, Jessica, Floyd, Cindy, Elaina, Steve, Robert, and others are patient,kind, and great at helping you recover. The ladys in housekeeping always do a great job.The only downside here is not a fault of theirs but the industry is food. It's not horrible but not great but then again when is it in a healthcare setting.
Orange Park Nursing and Rehabilitation is an amazing facility. The staff there are caring, professional, and knowledgeable. They treat their patience’s like family. My family and I are so grateful that my father spent his last days there, rather than at any other location. The facility was in a prime location, which made it easy for our family to visit at anytime. I 100% recommend this facility to anyone looking to have their family member treated with respect and dignity.
Horrible horrible place! They let patients go without food. They won’t answer call lights. This place is nasty! Where is AHCA??? Lots of bugs! Roaches and ants and who knows what else
