My loved one who has been staying on the second floor for over a week now has repeatedly received substandard care. She is recovering from a fall and can not make it to the restroom. Everytime she hits her light it's a minimum 15min before anyone comes to the door. She has sat in her own feces for over 30min waiting to be changed and today moved herself (she's a 2 person assist) from her wheelchair to her bed because though she hit her light no one came and the pain was to unbearable to continue sitting. I've had just about enough and have begun documenting everything and have an attorney on hand. I don't know what happened to staff since her last stay but this is totally unacceptable.

2/27 3am , Meanwhile dropping my wife off to work the security harass me beyond extreme measure. Upon arriving to the door, I usually wait until my love one is in the building safely. Instead I’m getting a knock on the window asking me to not park but I was simply dropping off someone to the door in 37 degree weather waiting for them to get inside. I drive away not knowing if she’s safe to find a parking spot to wait for confirmation that she’s okay. Another knock at the door saying that I’m trespassing and that I need to leave the property AS SOON AS POSSIBLE. I attempted to explain further that my wife is a NURSE and this facility needed someone last minute but the African American male with a short hair cut working the overnight shift driving a white Chevy traverse with a ride along partner with dreadlocks just continued to disregard what I was explaining and that makes this company look extremely bad.
My father-in-law entered Oak Trace last spring for rehabilitation after a stroke. He liked the staff and their concern for his health so much that, after rehab, he decided to stay in the assisted living area. The staff is very friendly; it seems everyone is always smiling. His room is clean, bright, and inviting. The food is excellent,,,,maybe too good for weight control. The staff's attention to his well being has been outstanding.
When the need arose to find an assisted living facility for our mom, my brother and I reached out to everyone we knew with similar circumstances. Over and over again people highly recommended Oak Trace. During our tour we were impressed with the well maintained grounds and the beautiful facility. We looked no further. We found Mom’s new home! But, it is the staff that are the Crown Jewels! Everyone from the front desk to the administrative office is friendly, helpful and knowledgeable. The caregivers are nothing short of amazing! The food servers, nurses, CNA’s, PT’s and OT’s are not only competent, but they deliver care with respect, kindness and compassion. Our mom is happy in her new surroundings. What a gift this is! I highly recommend Oak Trace for you or your loved one. It’s a great community!
I moved my 95 year old father into Oak Trace Catered Living today after 9 weeks of rehab from pneumonia, which was at Oak Trace, and 3 weeks in a step down room. I have never regretted my decision. The staff is so caring and accommodating. The food is great (we had a dinner with my dad there). Talking to the residents, each one will tell you how much they like life at Oak Trace. If you have loved one needing care, this is the first place you should be looking at.
