My MILove just stayed here for over 6 weeks to do therapy after an extended hospital stay. The staff, nurses, and therapist were so kind to her and really worked hard to get her better and well enough to come home. Joe is an exceptional physical therapist and really went above and beyond. My MIL never had any complaints about her care here and really appreciated everything Travis and Glen did for her during her stay. The facility was always clean and well maintained everytime we visited (daily most of the time). The residents seemed happy and well cared for and the food was a nice variety. They have an extensive calendar of activities and even an ice cream parlor. We had a great experience with all the administration and they were so knowledgeable about the entire process.

My Dad spent six weeks in the rehab unit of Moultrie Creek. My Mom and I spent many hours in the facility to provide emotional support to my Dad. I was very impressed with Moultrie Creek, Below are some key points: 1. Cleanliness The facility is very clean. The courteous cleaning staff is constantly seen about, cleaning the rooms, hallways, and common areas. 2. Nursing Staff The LPNs that managed my Dad's care were patient, kind, and knowledgeable. I would like to give a special thanks to both Lance and Andrea, who were outstanding and provided impeccable care. 3. CNAs (Certified Nursing Assisants) Very hardworking, dedicated and courteous. 4. Therapy Department The therapy department is exceptional. I would recommend therapy at Moultrie Creek to anyone. My Dad's therapists Athena and Jacob are top notch and were always kind, patient, firm and encouraging. My Dad made great progress under their care. They got my Dad walking again and taught him how to use a walker as a tool for stability. 5. Social Activities The caring staff engage the residents in many activities throughout the day. Beach ball, bingo, manicures, exercise classes, art projects, movies on the big screen, ice cream at the parlor are just some examples. 6. Meals The meals are large and balanced. You can also make special meal requests as well (although we didn't do that) residents can enjoy their meals overlooking the beautiful courtyard Although I hope my Dad doesn't have to return to full time rehab, if he ever does, I would definitely return to Moultrie Creek. When my husband needed skilled nursing, I visited numerous facilities and decided upon Moultrie Creek and have never regretted my decision. My husband has been a resident of Moultrie Creek for over a year and I personally am here everyday as well so I see firsthand what goes on. The facility is clean and maintained. The nursing staff and CNAs are very caring and many have been here for years. Jason the executive director is visible in the halls and very approachable. Care plan meetings are held quarterly with family and staff members and they work with you so your loved one always has the best of care. Complaints are addressed and resolved quickly. Dietary is very helpful and together you plan your loved ones meals... Likes and dislikes, etc. There are daily planned activities including musical entertainment and ice cream social hour which is very popular. There is also a beautiful courtyard if you'd like to sit outside and get some fresh air and enjoy nature. It's never easy to place a loved one into a nursing home. Alzheimers is a very difficult and long journey to travel alone but when I come to Moultrie Creek I don't feel alone. The knowledgeable and friendly staff have come to feel like family. I know they're here for me and my husband.
Moultrie Creek is a beautiful building with extremely hard working and caring staff. Just looking at the residents, it’s obvious that they are happy and feel well cared for. Would definitely recommend to anyone looking for a caring facility for their loved ones rehab or long term needs.
I would not recommend this facility to anyone who needs a nursing home. The minute you enter the main hallways of the facility you are overcome with the smell of feces and urine. The facility is filthy. The bathrooms are particularly gross. At times I would enter my father's room and my feet would stick to the floor. This is most pronounced on the weekends when most of the staff calls in sick. There are very few competent staff members. From what I have seen, the physical therapy group is very efficient and competent, the nursing home support staff is not. My father was there for 2 years. Two weeks before he passed he was in horrible pain. I had to constantly ask for the nurses to give him something. They told me his medication was to be provided "as needed" in which they seem to interpret that it's not needed because they do not check. It takes 30 minutes, many times longer to get assistance after ringing a call button. He passed before they got around to performing rounds. He was on his death bed, but no one was paying attention. Several times my father was given drinks with his meals, but he needed to use a Kennedy cup and no straws were provided. I brought my own straws or I had to go to the nurses' station to get them. Residents who can't feed themselves can wait an hour before being fed whenever the staff gets around to it. Residents are not changed as often as they need to be changed. There were many times my father sat for hours in a dirty diaper, just ignored. The only time you actually see the facility clean and running somewhat efficiently is when they are getting an inspection or corporate is visiting or they are giving a tour.
