I cannot recommend Lone Star Rehabilitation and Wellness Center in Stephenville. My mom was there twice, once at the end of 2023 and again at the beginning of 2024, and both experiences were deeply troubling. The meals were inedible, with staff dismissing concerns about her nutrition, despite her doctor emphasizing its importance for recovery after surgery. Basic care, like baths and wound care, was neglected, and physical therapy appointments were often missed due to being “too busy” or understaffed. The most devastating issue was their failure to properly care for my mom’s surgical wound. Despite numerous red flags, they overlooked clear signs of infection. When she went for a follow-up with her surgeon in Fort Worth, she was immediately admitted to the hospital with MRSA. This preventable infection quickly escalated to sepsis, and within a few days, we lost her. Her death was entirely avoidable if proper care had been provided. It’s incomprehensible how a rehab center, supposedly specialized in post-surgical recovery, could fail so utterly in basic patient care. While this place has positive reviews online, my experience raises serious questions about their standards. It seemed to me that their priority was profit over patient care, with any concerns met by quick, dismissive answers. My dad chose not to take legal action, so I share this review to hold them accountable and warn others. Healthcare facilities, especially rehab centers, must be held to the highest standards. When you’re paying for care—whether through insurance or out-of-pocket—you expect professionalism, attention, and compassion. We were quickly assisted at the entrance, after ringing the doorbell. The check-in kiosk was out of labels, but it did take our photo and necessary information to log us as visitors. The kiosk was easy and gave clear instructions on its use. Staff from entry to the residence, room, were pleasant, friendly, and asked if we knew where we were going, and the name of the person we were visiting. I am observed how all staff that we passed, took a moment to look at my husband, and I, for recognition purposes. The grounds and the facility are well-kept; staff in the hallways, going in and out of the residents rooms. As we walked down the hall to our friend, we were asked if we needed any assistance in finding our friends room. As we visited with our friend and his family, we observed that he was well-kept, his room was in order, he had no unmet needs. The family shared that they were very pleased with the facility, the staff, and everything that has been done since admittance. If I knew anyone who needed a recommendation for a rehab center, I would not hesitate to recommend this one. We will be back again soon to visit our friend.
My father is here. The facility is nice. Several of the residents smiled back at me as i walked by their rooms. Thus telling me it's not so bad. It's hard to leave a loved one in a strange place at first. The transition time is hard for the whole family. I believe it will get better as he gets the care he needs. Praying for my father my family and the nurses and staff to be patient understanding loving. This is the start of a different journey in all our life's.
Took a tour of this facility and I was absolutely blown away! The staff was very friendly and welcoming. It was very clean and the residents that I saw looked very happy. Shirl was very helpful with all of my questions and made me feel right at home. I definitely think this will be the perfect place for my mother for a long time. I was happy to see several residents engaged in activities during the time I was there. Whatever they were cooking for lunch smelled so good! Looking forward to having my first meal with my mother at her new HOME in a few weeks!
Today was awesome, it was time to go home. My mom enjoyed physical therapy, the staff was awesome too! Only rant my mom had was the food, you didn't get what was on the ticket, but even so she like some of it, french toast and grilled burgers were the favorites. Thanks for caring for my mom when we couldn't.
