Life Care Center is a wonderful facility. I went there for rehabilitation for my shoulder, and had wonderful care. The physical therapy department is first rate and the activity director is the best I have ever seen. She goes above and beyond to make sure the residents are happy and living a good life there.

Hello, This morning, I was told that my mother would be changed and properly taken care of. However, when I returned around 7:30 p.m., I noticed she was still wearing the same gown from earlier in the day. It appears that no one had changed her clothes or provided the personal care she was supposed to receive. I’m very concerned about this and would like to understand what happened and ensure that my mother is receiving proper attention and hygiene care on a daily basis. My father has dementia and spent about 8 days here after he was initially released from the hospital post infection. I was always very impressed with the staff (Allison, the CNA's, nurses, and the therapy team were great). My dad was clean and well cared for. My only advice to families is to also consider a Rehabilitation Hospital vs. Skilled Nursing. I believed my dad needed more than one hour a day of therapy and in our case, I made the right call in moving him. Still, if you need a skilled nursing facility, I would not hesitate to put your loved one here.
Since Life Care has had a change in management it has gotten so much better 😍 It has taken a little while but it is really showing. The only downside is there is a big turnaround of help and it is hard to get people that seem to really care sometimes. My mom has been there a year and a half and she still has a nightly visitor (not a welcome one) that comes in her room and sits down next to her and eats her snacks! I understand that they can’t bar up the other patients but there has to be something to do. My mom did tell me though that this other patient has someone watching her more closely and it has helped. But every one there is really nice, there just not enough people for the patients they have 🥹
My father with dementia who is oxygen dependent was a patient and we were told he could not have bedrails or a bed alarm as these are considered “restraints” which are forbidden.but they are able to put mats on the sides of his bed, which were not secured to the floor as one of them went out from under my feet when I stepped on it. He also came there from the hospital with a Foley Cather that they could not remove; it would have to be removed by a urologist that they would transport him to when we make the appt. I came to dress him for his appt and he was asleep and unresponsive. The CNA said he walked the floor all night and would probably sleep thru his appt. I immediately noticed he was not wearing oxygen; how long he was without it I did not know. They transported him anyway without checking his vitals. When I saw his condition at the urologist, I had the driver return him to Life Care. By the time I got there, he was back in his room and they were pumping oxygen into his lungs with a “non-rebreather mask”. I was asked by the nurse manager “why did we allow him to leave like this”? I reminded him I am not the medical person. I was told by the CNA he would probably sleep through his appt. He was transported to the hospital and as a result of this negligent treatment at Life Care, he is dying.
