I received very good care at this facility. They have an excellent therapeutic team, they were very encouraging, and motivated me to make it through the exercises. The aides were very respectful and mindful of my privacy. The facility was clean, I only have good things to say about the 5 weeks I was there for rehabilitation. I would recommend this facility for rehabilitation.

My dad was placed here when it had first changed to Legacy (from Pro-Medica). It was not perfect, but decent, and the administrator Pam and the staff addressed issues as they were brought up. The Legacy came in, and it wasn't long before a lot of the good people were let go or quit, including the administrator. My dad wreaking of urine and feces became common place. The facility was running out of diapers, the food became terrible, and the general attitude of the staff (save a few good ones that were left) had soured. Then, they changed they way they dosed medicine, giving a long window of time. Soon, my dad was becoming lethargic and drooling on himself regularly. We began complaining that he was being overdosed. The staff insisted that this was just a condition of our dad's disease - regardless of at least one incident in which it was admitted he was overdosed. We finally had to move our dad because it got so bad. He has been in a great place since June, and not one time has he become lethargic and drooled on himself. Shame on the people who would not listen to us and address our concerns. I would not recommend placing a loved one here - my dad was there a little over a year, and the decline in professionalism and staff became really bad. Sad that the State of Ohio allows this type of stuff to continue. Edit: Reply to Owner - I contacted corporate when my dad was there, and things only continued to get worse. I am not wasting my time reaching out now.
The staff took great care of me. I enjoyed the carnival with Angie. Therapy staff like Steve and Rich , Andrea help me a lot. Overall great stay.
Our company did work for this location and have yet to be paid. I hope the patients don't receive the same care. We have been waiting for payment since Sept. 2024. Needless to say, this is not an establishment that can be trusted.
This place needs a major cleaning and update. Very disappointed my Mother had to stay here. Her room was definitely not cleaned prior to her arrival. The bathroom was shared with the adjacent room. It was never cleaned. Base of toilet was black. I had to clean the bathroom for her to use. Flooring was buckled up under toilet. Floor in her room was sticky. The divider curtain in the room was dirty. Tiny tv on a swivel arm. She could not even see the tv. When I first arrived at the facility the grass had not been mowed. It was knee high. Carpet in hallway is filthy. I saw dead bugs along the hallway as I walked in. I feel bad for long term residents that live here. It’s sad the owner does not put money back into this facility. Thankfully most of the staff seemed nice. Tara and Alexa are absolutely wonderful nurses.
