Mom has been a resident of Mansfield Memorial Homes (now JAG Healthcare) for 2 years and has continued through out to receive watchful, compassionate and skillful care. I live out of state so rely in extra measure on JAG's staff in providing for her physical, social and emotional needs. In addition to communications I receive from mom, I receive ongoing updates on mom's status and condition from JAG's staff that assist me in making decisions concerning her ongoing care. Darlene Dale (and other staff members) have been especially gracious and helpful in assisting me in so many ways. I realize there are many challenges any Assisted Living and Skilled Nursing facility can have and I'm grateful for JAG's efforts to meet these.

My daughter caught a nurse stealing narcotics. And she turned her in. And 2 weeks later my daughter notified state because the facility and Marty and the DON ignored her requests to remove this nurse. My daughter is a great aid. Always has been and will be. They let her go. And now she is not rehirable. Kinda funny.
DO NOT TAKE YOUR LOVED ONES HERE. The facility is old and smells. The staff is HORRIBLE and unprofessional. They do not provide good care. My mother in law was dehydrated and required hospitalization several times while in their care. She was sent out to appointments without as much as a washed face. She was terminally ill and they did the bare minimum care for her. And when she recently passed away the staff really treated my husband awful. Didnt even offer condolences at first. THEY ARE THE WORST IN THE AREA.
Not a very good place for joint replacement rehab. I spent 9 horrible days there after bilateral knee replacement. This was the so called, "best" place that the worthless social worker at Ohio Health claimed they could find. I have very good insurance that pays up to 30 days of nursing home care. Food was awful-lost 10 lbs in 9 days. Couldn't even get a decent cup of coffee. Had 1 shower during my stay & it was like pulling teeth. Had to beg, REPEATIVELY for my ordered pain medication. Also had to beg, REPEATIVELY for ice packs to place on my knees (a basic standard of care for knee replacements). I was basically left totally alone & uncared for I have never been so afraid in my life for my well-being. I will never walk willingly through a nursing home door again. On a positive note the PT department was fantastic. They worked with me daily & tried to take care of the things the nursing home wasn't. Their, above & beyond, efforts kept me there the 9 days I stayed. I returned home too early & relied on the kindness of strangers to get me thru until I could care for myself.
Beautiful historic Mansfield campus. I see their team truly treat their residents like family. I often see staff go above and beyond to address all resident needs, holistically. The warm atmosphere feels like home.
