My husband spent 5 weeks in this SNF primarily for rehab. I met with the administrator, Angela, prior to his admission & she was highly knowledgeable & transparent re: services provided & I was able to tour the facility prior to his admission. His room was large & clean & the staff was friendly & responsive to his needs. The therapy provided was in his room or the dept on a daily basis. Staff was generally available for questions. I would recommend this facility for rehab & care prior to transitioning to the home setting .

Embassy of Lyndhurst is a great facility. They treat my mom great and help her with all her needs. They take that extra pressure off me know that they take great care of my mother and the communication is great as well
THIS FACILITY SHOULD BE CLOSED DOWN. Visited a loved one, they had the head of the bed down and his legs up so he couldn't get out of the bed. CNA had on street clothes. Never saw the nurse in my 45-minute visit. The CNA working told my family to take him home, they were just taking our money, which was an inappropriate conversation to have with us, in the presence of their resident. I had a resident tell me how to hit the buzzer to get out the building. EMBASSY OF LYNDHURTS IS A SMELLY, OUTDATED, UNDERSTAFFED TERRIBLE PLACE. DO NOT PUT YOUR LOVED ONE IN THIS FACILITY!
My mother came here about four weeks ago and the experience has been horrible. She was given no information about her progress after a broken hip and leg. I repeatedly spoke to administrators with promises of phone calls and updates and got nothing. The nurses and physical therapists were fine but administrators do not follow up with patients or family members. She missed her first doctors appointment for follow up because they forgot to call to schedule it. Another patient in there with her said the same thing happened to her so this is clearly not an isolated incident. They didn’t help shower her for 11 days after her surgery and that was only because I went in and yelled at the front office, even though they have posted signs in each room about showers every Tuesday and Friday. When she was finally discharged from the facility nobody helped us or her to the car. She was told that there would be physical therapists and nurses aids coming to her home as part of her rehab … It’s been three days later and no one has called her or come by the house. I have tried calling the social worker and Jack, the person in charge at the Greens...no replies. I am so disgusted with the treatment we have received. I wouldn’t recommend this place to anyone. Shameful the way my mother was treated.
If you love your loved ones do not put them in this place. I wouldn't put a pet under anyone's care in this facility. I have a family member that went in there on a Friday and was taken back out by ambulance on Sunday. They did not give him his medicine for 2 days. Never gave him a breathing treatment left him to sit and wait to have a bowel movement for hours not responding to the nurses call. When they did finally get him to the toilet they would leave and leave him sit there for at least a half hour. The kitchen staff had an out and out physical fight outside the building that police were called to. the room stunk of urine if you called the facility from home you get passed around until you end up to a phone number that no one answers there was nobody higher up to call and complain to nor has anybody returned a phone call yet. The food they serve is not a nutritious well-balanced diet you get hot dogs for lunch and hot dogs for dinner he never even received a dinner the last day that he was there. I could go on and on but I would figure you get my point. And it is my mission to try and get this place shut down. You don't treat human beings like this.
