My 93 year old father was admitted yesterday. He was greeted be two of the nicest STNAs. They made him feel welcome and comfortable. His Nurse for the evening came right after and did his assessment and also made him feel comfortable and welcome. If Dad is happy and comfortable, we are.

All of the people working at Green Meadows are so nice and helpful. Rachel in particular is very helpful and I feel really comfortable with her. Having my Mom there has taken a lot of worry off of me and I know she’s safe and taken care of. I have absolutely zero concerns or complaints with Green Meadows and their staff. I would like to thank everyone there for all of their hard work and dedication.
My father was here for rehabilitation. Physical therapy group was very gentle and didn’t push him too hard. The nurses are nice. Seem short of staff. Takes up to half hour to get help going to the bathroom. Getting much needed pain relief or changes in medication on the weekends and nights is almost impossible. Water is brown tinged and tastes horrible. Ceilings look water damaged.
Green Meadows is clean and nicely decorated. The staff is pleasant and helpful. Our relative is well cared for and we are kept informed of any changes or reports concerning his care. I recommend Green Meadows Skilled Nursing and Rehab of Louisville, OH.
A very clean, organized place to work as well as live/heal. Administration is very hands on, working hard to effectively ensure the wants/needs of clients. If you work here, they educate with grace, understanding and patience. These administrators care beyond their scheduled hours and always make themselves available to help. The units are organized based on needs of the residents which makes care organized and intentional. Wonderful therapy team. PT/OT communicate directly with the nursing staff to meet the goals of the clients care. Green Meadows has been around for YEARS. The building itself is kept clean by everyone who works. Maintenance requests are fulfilled quickly. Housekeepers work around the clock most days and nights. In house hemodialysis is priceless to the and staff. Overall, the communication of ALL departments amongst each other makes Green Meadows a step above every other facility in my opinion. If I could give it 10 stars I would.
