The therapists and front desk staff are the only positive thing I can say about this facility. It looks nice. Obviously it's been renovated recently. Too bad they didn't spend money on training and hiring competent and caring staff. I'm more than disappointed in the administration and nursing staff. When the call button is pushed because the patient needs to go to the bathroom, it could be an hour to an hour and a half before somebody shows up. When you try to hunt somebody down, the nurses are typically sitting at the nurse's station chatting about their weekends or scrolling through their phones and don't even acknowledge you. There's no consideration or assistance provided for patients who have motor control difficulty and can't feed themselves. The staff will place the food in front of them and leave. It's been weeks and there's been no interaction between the family and case manager regarding discharge planning.

During our visit, we had the opportunity to interact with Sophia, a CNA working the floor. We felt compelled to reach out and share our sincere appreciation for her exceptional care. Sophia went above and beyond her duties in providing support to the patient, demonstrating a level of compassion, professionalism, and dedication that truly stands out. It was evident that she genuinely cares for the well-being of those under her care, and her efforts did not go unnoticed. We wanted to take a moment to acknowledge her outstanding contributions and to express our gratitude for the difference she makes in the lives of patients and their families. Jan 5 entered Jan 29 released As you read my feedback, know that my dad once upon a time had mini strokes, so BEFORE this incident and entering the facility he was unable to do several things himself or needed assistance. His right arm and leg were very weak. No one asked or knew this prior to him entering..and no one asked during his 3 1/2 weeks he was there. We had to bring it up to help the therapists understand his needs. On arrival date, it was evening time and my sister found my dad laying in a fetal position on the edge of the bed with only a gown on. Apparently, the transporter said his shift was over and someone would be in. On several occasions on different days, he waited over an hour to use the bathroom then finally we helped him. Yes, we timed them because there seemed to never be urgency or the excuse was they were short staffed. One staff member told my dad to start yelling for help after 15 minutes if no one came to help. They didn’t cut his food regularly, which was supposedly on his chart. I started taking pictures for proof it wasn’t being done to show them. They also, forgot straws and to pour juice in cups for him. He needed help eating most meals and when he asked them to cut his pancake the lady told him to eat it whole. One evening I was there they forgot his dinner completely and realized it when they came in to pick up the tray that they never even dropped it off. Jan 16, we finally asked if he had a social worker and got her contact information. We tracked her down after calling and going to her office with no response back to us. When she was finally located in her office, she looked up my dad in the computer and said looks like 4-6 more weeks, but never even met my dad or knew anything about his prior situation before entering the facility. On two more occasions we had to track her down to ask what was going on because again, she never came in or called to tell anyone the plan. In fact, no one at the facility ever asked his history, so how could we make plans when he was released if they didn’t even have information on his home needs. Up until the day before discharge the 3 times we spoke to the social worker was when we tracked them down and it was always a hallway discussion. Jan 22 I tracked down the Occupational Therapist, who’s an AMAZING lady.. she helped us with our questions and we came up with an exit plan together. While there, I had to cut his finger and toe nails after one nurse dropped off the clippers without doing anything. The next morning the CNA made a comment about how “they left her to do the job”… but at the end of day two, I did them myself. Another occasion he had really bad back pains because on the weekends when there isn’t therapy, they just get plopped in a chair and the tv gets turned on. Even after I brought in a lumbar pillow to help him he asked for Tylenol due to the pain. We asked the CNA for some medicine and waited 45 minutes, then I had to track them down again to make sure he got what he needed. Lastly, I’ll add that on several occasions when we arrived to visit around 11:30 on the weekends, he was sitting in a shirt and underwear in the chair with no blanket. No one offered to help dress him or cover him up. How demoralizing! Luckily for my dad, we had family there everyday! While we were there, we got him dressed, put him to bed or back in bed, brought him food at times, & took him to the bathroom because the service was slow or ignored. I won’t even get into the debacle with the home nurse care when he was released that also got messed up … but after a week 1/2 finally PT came, and the others are waiting to be scheduled
My mom was there for 11 days and out of the 11 days, they couldn’t control my moms sugar, they would give her normal food, no diabetic food, no sugar free anything, my mom had to call me to let me know that she was not feeling well, my family dropped everything and went there, my mom was yellow, lethargic, slurring her words and was in and out of coma, they refused to call 911 on my mom, so we did, when we wanted to discharge my mom because they refused to call 911, the one nurse stated to the charge nurse “they want to discharge her and take her home” and the charge nurse said “why so she can die?” Not knowing my wife and I was standing there at the nurses station, we had mom sent to the hospital and my mom was in ICU for 16 hours, my moms room was dirty, not clean at all, they have a few staff members that was extremely nice and helpful BUT overall, it was horrible and I will NEVER recommend any of my love ones to go to this place!!! It’s a joke!!!
Fountain Hills Post Acute has the best Staff ever! Literally the BEST Rehabilitation facility! It’s been a blessing being a part of the community here and working with the staff here. It’s been like family! Thank you guys for everything. Highly recommended!
