Who decides, are you included and updated on your family members condition? Ask questions! 1. What training is required of direct care staff? 2. What is the education level and experience of all staff? 3. What dementia care training is required? How much is adequate? 4. How would staff handle resident to resident violence? 5. Ask about frequency of medication errors. 6. Ask the ratio of residents to staff. Ask about this ratio on all shifts, in your mind- is the number adequate? 7. Ask about staff turnover. 8. What services are actually covered in the basic monthly charge? 9. Does a licensed nurse cover patient assessments on weekends and after hours and does this include resident visits or only phone consults? 10. Can this facility safely care for my loved one if their condition changes? Do they add additional staff if there is a change in the acuity of care needed in the resident population? 11. Describe the facilities role in end of life care. ( Hospice is a separate entity) 12. Ask about pressure ulcers and skin breakdown. Be prepared to; Visit often and monitor the care of your family member Visit at different times of the day, evening, and night to observe and ask questions. Visit at meal time Our lovely family member's health declined rapidly while in the care of this facility. We experienced system miscommunication, medication errors, and a broken hip for which we were not notified. The resident assistants were all very kind, hard working, and concerned about resident care. We believe our experience is a result of system errors and lack of adequate leadership. Ask questions of any facility that is entrusted with a family members care.
