My mom was at Centennial House Assisted Living for 15 months before we moved her to be closer to where we had recently moved. We loved the care attendants, the activities staff, the PT lead, and all of the leadership. They consistently showed caring compassion and empathy toward my mom and all of the residents every day. All through the COVID experience, I felt that my mom was in the safest place she could possibly be. Protocols were implemented promptly and efficiently even before there were state mandates! Thank you Centennial House! We will miss you all tremendously!

My mother lived in Centennial House for the last years of her life. She just blossomed there, regaining her interest in living and activities! This was due to the kind and caring staff who take interest in all the residents, the wide array of activities from which she could choose, the calm atmosphere and stability and safety of a secure residence. I was able to have peace of mind knowing that she was "watched over." When medical issues arose (and they did), I was notified and the Bluestone care plan was immediately accessed. It was the ideal situation for both her and our family. We are forever grateful she was part of the Centennial Family!
The reviews posted do not fall under the Ecumen House my mom has been living at for the last 10 months. The Memory care is not this rainbows and butterflies as these reviews led you to believe. They have revolving door on staff never know who is working there. Activities? not sure what they call an activity other than placing my mom in a chair and turning on a tv. We actually have to pay to have someone go and take her for walks and such three times a week. We constantly have to chase down her missing items. Clothing goes missing almost weekly. Falls go unreported. Mystery bruising shows up without notification. Rarely get a response to phone calls or emails. They pass the buck to other departments. I was bamboozled with our walk through. We were promised the moon and got poison in its place. I would not recommend if your loved ones need assistance with dressing, showering and using a bathroom this is not the place for them. The assistance with helping them dress is also lacking. I have gone in and my moms is not wearing under garments. I am toileting her because the urine smell is outrageous when I visit her. I have attempted to write these reviews previously but you are only allowed to post good reviews. Just so you are aware. There is not enough staff in the memory care to care for the residents.
My aunt spent the last eight months of her life as a resident at Ecumen Centennial House in Apple Valley, MN. All phases of the healthcare she received were excellent. The resident assistants, the nurses and the hospice team all provided compassionate, loving and personalized care for my aunt. The facility was always clean, bright and well-maintained. The three daily meals were always served on time. My aunt said the food was fresh and delicious and the staff was very good about providing mealtime options for the residents. Resident events and activities were fun and enjoyable. The Centennial House director, marketing, and business office staff were very cooperative and supportive. We were very fortunate to be able to find a facility like Ecumen Centennial House. We will always be grateful for the marvelous care my aunt received at ECH.
My Dad lives in Memory Care he is provided great food with a mix of choices and goes on walks along with play chess his favorite game. Nothing but good things here. Great tip you need to mark the cloths because if they are not marked they will get mixed up. It is very clear upon move in you need cloths marked. I notice everything is very clean every time I go to visit. I am so great full for the wonderful staff.
