I am always thoroughly impressed with the staff and the compassionate care they give the residents. The facility always looks great, there is always an activity going on. The staff care for these residents like they are their own family members! The environment is always clean and comfortable.

Ok I spent a month there in overall that's not the facilities care is very very good. Physical therapy as well as the occupational therapy was very good compared to other places I've been in the nursing staff there were very very friendly as well as very responsive. You didn't have to pray that the nurses would eventually show up for a call flight The only problem there is at the food was very very bad. But that's par for the course The only other problem is that the CNAs could do better in terms of for helping out with personal care When you had to fall and you have problems with mobility and walking and you have a big sign saying fall risk tattooed on your forehead it's hard to take care of yourself
I hear nothing but Bad care, bruising, leaving marks & neglect (not opening cereal) from residents. Why would I want to put my family in a place that uses the excuse that short staffed to not do their jobs. Sounds more like Lazy Union to me!! I will make my own judgment in the spring!
The only reason I do not give the rehab one star, is because the nurses were great. I have to say I was disappointed that an aide told my husband that he was lazy. Very unprofessional. Rehab amd communication is subpar in my opinion. My husband has been in other rehabs in these past few years so I speak from experience. At DeGraff, He had one PT and one OT session per day. It was decided to do them both in the morning, which resulted with him sitting in a chair all day with no one walking him in the afternoon. He was not allowed to walk to bathroom. He was discharged early without any input or discussion with myself. PT really had no baseline info on what he was able to do at home. My husband barely made it into my car and when I arrived home, he could hardly walk and needed assistance. He fell again later that night. There was no discharge planning except to tell me they had referred him to home PT without asking my preference as to which PT Association I preferred. Finally, there is no way to talk with the rehab unit as there is only one phone number to call and that is answered by a voice mail telling you to leave a message. There is no way to communicate with the rehab floor itself or any other department or personnel. I have been an RN for 40 years and am well versed in several situations, so, I feel qualified to voice my concerns
My mom has been in Degraff SNF for a little over 3 weeks. The entire staff has been great, they are taking such good care of my mom !! I can't say enough about how attentive and caring and dedicated they all are !! The rooms are very large and very clean and bright !!
