I never leave negative reviews, but this place is not good. There are some very caring staff, but they are few and far between. My dad has been there needing rehab for his legs and once they are good, he will return home. His PT is 30 minutes per time (maybe 3x a week) and he isn't making much progress at all. The place is dirty, the care is extremely slow. They say the are fully staffed but my dad has had to wait HOURS (on most occasions) to get someone in to assist him with anything. One night he ended up in bed sitting in his urine (needs assistance to go to restroom) and it took HOURS for anyone to answer his call light to help clean him up, etc. I found out he isn't getting routine hygiene and had to press the issue. I am currently in the process of looking for another rehab facility.

When my mom was going to go here for rehab, we read the reviews, and were a little worried. However, I have been really pleased with the care that she received. There are some absolutely amazing, patient, and caring staff. The PT, OT, and nurse practitioners were all amazing! We also had some really great CNAs, too. I’m glad we went in with an open mind!
I have.a Friend and a relative here.long term. The place is filthy and sheets only get changed when asked by family or friends, no matter how soiled they are. There are dirty bandages on the floor and the water cup at bedside is over 2 days old. Bathing patients is optional here as well. Personal laundry goes missing and as such family has to do the laundry. Food is unidentifiable for the most part and served cold. Breakfast trays remain in the room until evening meal. Meds are left at bedside instead of watching patients take them as I found a morning med cup full of pills past lunch time. It is not uncommon to have your call light on for up to an hour before being checked on and when checking on staff there are 4 people sitting in the nursing station talking. Daily wound care is done whenever remembered by the staff as dates on the bandage have been noted at 4 days out. Place is unsafe and unsanitary..

After a week long hospital stay, and hours to make a decision on what rehab facilities were available, I chose Crossroads for my mom. Upon entering, it smelled like urine, walls were full of grime, and the facility was obviously in disrepair. I tried to overlook some of these things, with hope that the care she received would be better than the living conditions. Among the things we experienced: even though they told us they have a twice a week shower schedule, she was given ONE shower in two weeks. It was only after formally complaining three times that she was actually bathed. She was lucky if a CNA responded within an hour, if at all, when she needed assistance. She went days without having help to brush her teeth. She sat in her own s%$t for hours, because she was given a suppository and had to "go". After calling the nurse call button, no one responded. She was never offered the option to leave her room to go outside, or any basic social needs. She was disregarded as a human being. If you don't care about your loved one - this is the place to be. If you love your family member, 1000%, choose another option. The ONE positive thing I will say is that there IS some good staff here - primarily the PT staff. They do care and I was pleased with her therapy. Other than that - this place is the biggest dump I've ever encountered and I'm shocked they are allowed to operate.
Unfortunately zero isn't an option. Nurses walk around with their cellphone on their face all day or talking at the station instead of taking care of their residents. Leave folks for hours in wet clothes. My aunt was made to sit on a toilet for almost a half an hour before she was even checked on. Takes almost an hour for call lights to be answered. There are a few really caring nurses and staff members but way too few. Get your staffs act together.
