Creekside Village Assisted Living 1000 E Stuart St, Fort Collins, CO 80525, now has new ownership out of Utah; Cottonwood Healthcare 947 S 500 E, Suite #105, American Fork, UT 84003. They are currently remodeling and opening up for additional clients. Creekside Village Assisted living admissions at 11:15am on 3/23/2025; Intake nurse was not informed beyond the Plan of Care that I detailed and provided in advance. 2.5 hours later a Supervisory nurse signed off on medications, causing substantial delays in medications being administered and a very late meal. Wi-Fi password was unknown to staff. 7 hours later at shift change, one CNA was able to supply that Wi-Fi password. Dinner was delivered an hour before the required rapid insulin could be administered. Microwaving doesn't help texture. Meal trays never were removed from the room. Maintenance issues; No working nurse call button, no working TV remote, no way to silence nurse call alarms in hall, no electric Hoyer was made available until 10:15pm which cannot access the bathroom (a portable commode was brought in from home in addition to our lift chair), no trash can, paper towels in bathroom dispense on their own, toilet water runs occasionally. An overly firm mattress made sleeping uneasy. Urine smell from some rooms is unacceptable, so we keep the door closed. No ventilation except windows adds to the discomfort. No odor mitigation is permitted by client. Clarity is needed about prior listing of pharmaceuticals and known allergies, with sign offs by Creekside pharmacy in advance of arrival. CNA staff needs to be trained on using PureWick external catheter use. Human excrement was removed from under the heating ventilation below the window! Then the numerous repairs are needed as evidence in the attached photos of room xxx.
