This place is a joke. They allowed my grandma to get so sick with sepsis that they finally sent her to Winchester! We have tried everything to get them to listen and demand treatment. When she finally was 100% cleared to go back to the consulate she was given her medication as directed for hours. Then she passed away. The amount of time our family spent yelling at them and the social services department in there is ridiculous! It’s hard enough to make a decision to have to put a love one in a home, then they let down their part of caring for her!

The food sucks. Staff ignores requests for help from patients even to go to the bathroom to relieve themselves. My brother was a patient there and was made to wait for 4 days for a bath and change of underwear. He requested, But was ignored. Do not pick this place for your family member.
I could only rate this a 1. It wouldn’t allow me to give zero. I am currently in a resident of Consulate at Woodstock. as of May 1, 2025, the name will change to Woodstock Valley health and rehab but be assured nothing is changing just the name. I currently was the victim of a founded APS investigation against the director of nursing for intentional mental abuse along with multiple rights violations. This facility often runs out of your medicine, causing sickness, pain, and other problems. There are many other problems here at the facility and repeatedly have gone to administration, executive director, corporate with no answers and no solutions things only seem to get worse. They run out of food , Don’t serve the full meals they’re supposed to. Multiple times not given protein during meals.. And after all the complaints by myself, my family and others nothing has been done to fix the problems. It continues to happen. I’m told the budget only goes so far to feed us. Told Buy a text, by a kitchen manager that he knew it was poor quality, but that’s all they could get .pnot served fresh fruit. Recently was ignored by medical professionals, namely the PA in charge of caring for us for over a month. Repeatedly, I was told she was leaving soon counting down the weeks, but it didn’t change anything about my medical evaluations. There were none many of our blood sugars aren’t checked anymore leaving a question of what are our blood sugars who knows because nobody’s checking them I just found out yesterday that the director of nursing who abused me will be returning to work tomorrow. I’m disgusted and it is unconscionable that this woman returns to work in the same place and situation that has caused me so much pain, grief nightmares, and continued extreme anxiety. I wouldn’t recommend this facility for anyone, ever!!! There is even so much more than I’m going into here, but I would be happy to let anyone know what written in my notebooks, the pictures I’ve taken, the evidence I have to prove everything I say!. And as an example on April 29, 2025, they were missing half of the food on my breakfast list because they were out. How do you run out of food for a residential facility like this? I was The budget only goes so far. Here’s Pictures of breakfast and what if you can’t eat bacon the grits, cold and Congealed You can see the finger mark in them. Would you want your family getting this? Heidi Teabo
I've recently placed my brother here. He has been through so much and has been relocated way too many times. I finally feel that he has found where he belongs. The staff here are incredible. Very helpful, respectful and caring. The facility itself always appears to be very clean and fresh smelling. The rehab department is knowledgeable, friendly and caring. They patiently work hard to keep the residents moving and motivated. I would definitely recommend this place for your loved ones.
This is a little long, but it is all true .- I had previously heard bad things about this facility, however wasn’t sure it the things i heard were accurate, I was looking for a facility close to home for my mother & there was limited availability at the time. When I first inquired about this facility for my mother, everyone seemed professional , kind & caring. In June of 2023 my mother was admitted here into the long term memory care unit. Of course the transition for her was very difficult & upsetting as she just wanted to go home back to her day to day life. Due to dementia/ Alzheimer’s & being a diabetic she needed 24/7 supervision. I will say there were a few staff member that care , are knowledgeable & try to take care of the residents & patients. There were also many that either just didn’t care, or have not had proper training.! They DID NOT give proper care, I’m so many ways, she declined rapidly cognitively & physically, she appeared to be sedated many times I was there which led to cognitive decline. I questioned the nurses & director of nursing as well as the administration many times actually every time I was there which was several times a week! My mother fell, they never notified me. They would let her sit outside all day with no water. She had numerous ulcers that she Never ever previously had. She had cellulitis in both legs , she got where she could hardly walk or talk. When I told them I wanted her sent to the hospital for care, one of the director of nurses & the nurse practitioner asked me why? I insisted, & I took her to the ER where she was admitted & spent a week in the hospital for multiple reasons. Dehydration, uti, multiple ulcers, cellulitis in her legs, sugar over 600, Calcium and sodium levels. & later found out she had mersa after the hospital test came back. She couldn’t get up out of a chair/ bed herself or walk without assistance she was so weak. I had her transferred to a different facility.. all her infections have cleared, she has gotten strength back, walks constantly daily. Ulcers, cellulitis & mersa cleared. She’s Not on any medication s that she was given at consulate! We honestly believe if I didn’t take her out of consulate when I did ( September 25th, 2023) that she wouldn’t be with us today. I’m not sure what was wrong with those in charge, at the time but there supposed to take care of people not try to make them worse! There was a few that tried but several that probably should get proper training or get in a different line of work! A couple nurses that do not have the patience or demeanor to work with dementia patients! & anyone without medical training could see there were medical issues that needed attention so what was wrong with the higher up nursing staff & up that didn’t seem to notice or care?? Also in the 3 months or so my mother was there, I noticed A few changes in Staff, Different Positions, In the office areas, Director of nursing and admissions. I also noticed at least one of the staff members on A few occasions Looks like maybe they were on On something. On another occasion, when I was leaving , A patient that was there for physical therapy told me how Place was and that they would not even want their dog The way they were treated. I would not recommend this place. I wish I had checked it out more before allowing my mother to be placed there., I definitely feel that they contributed to her rapid Cognitive decline. We feel Because of the care she should have received and did not, And with the combination s of medications, We honestly believe that caused her to cognitively decline so Rapidly. The disease itself dementia and Alzheimer’s is a horrible disease We Believe because of the way things were handled during her time at It made her condition worse, She declined so quickly there , We probably lost The mother, we knew much faster Then the disease itself due to the treatment she received at consulate of Woodstock. I have pictures and videos of my mother when she was there. I really Hope things have improved a lot for the patient sake.
