Please do not consider leaving your loved one to the care of Concord. They lack the knowledge on how to care for patients with dementia and instead leave them to decline. They made a lot of promises of high quality of care when we took a tour of the facility but the actual care received was nowhere near the promises made. The nurses and aides always acted like you are a bother when asking questions or for updates on the patient’s progress. My mother had a broken hip and the aide that came in to change the bedding didn’t read the chart and was about to flip her to change the sheets. Do yourself and your loved ones a big favor and go somewhere else.

Although my father only had a short stay at Concord, I was impressed. The nurses and staff were attentive, kind, and took very good care of him. My father was calm, content and happy. I believe that was because his needs were met in a timely manner.
My son was just admitted this afternoon and this was my first visit. So I really haven’t had enough time to have an opinion. You can ask me again in a few days. I did find the nurse Jacob very attentive and pleasant. The environment and decor is outstanding. What I have seen so far deserves 5 stars.
Concord and its staff are amazing. From Josephine who was available 7 days a week, texting from home to assist my concerns, to Judy, Kim and all the nurses and Social workers who were always pleasant and knowledgeable. The rehab helped my mother and got her back on her feet. Caring individuals
Concord Rehab.& Healthcare is an amazing facility! My husband Bob has been here for 18 months. When we arrived he could not talk or feed himself after a severe stroke! It was unbelievable after a few months, he was talking and feeding himself. The welcoming we received was remarkable! The Concord team greeted us and explained clearly what the next steps would be! They always kept us informed by care meetings. All the nurses, Prestina, Susan, Christine, Jacob, Debbie, Kim, along with the Unit Manager Kristi, who was so extremely helpful when things got really tough. Michele my previous Unit Manager was always there guiding and helping me out all the time! Heather (DON), treats all here patients with such excellent care. Everyone is top notch! They treated you like family! Josephine and Emma were so helpful setting up everything so smoothly for admission! They too, are so caring in every way and willing to help out in any way they can. Jay the administrator was always around making sure everything ran smoothly. He always showed his concern! Kristine, the dietitian made sure Bob always had the correct diet! Nicole, the social worker was always so helpful with any questions or problems and went out of her way to help! Judy the concierge also stopped by and visited Bob often keeping him with smiles. When I had surgery, she even went out of her way to bring him food he liked! Desiree was a pleasure to greet you as you entered Concord! She always has a great welcoming smile! The aids are wonderful too, , Keish, Diana, Mark, Kathy Ann, Wanda, Angela, Candy, Ann, Marie, Lisa and Whitney! Besides making you feel like family they are always there to help you out! I can’t say ENOUGH of what a great facility it is to have your loved one stay. Thank you to the Concord Team for everything!
