Reviewing again because facility did not respond to update of previous review. We moved our family member out of this assisted living facility for numerous reasons as listed in previous review and here: lack of care by some staff members probably attributed to by turnover of staff, unsanitary conditions, inability to get repairs and maintenance to family members unit, calls from poorly identified telephone numbers at hours that would lead to belief of emergency, however no messages were left by calling party, return calls to the number were not answered, voice mail that answered was only identified by an extension number and frantic middle of the night calls from our family member. She was often left in her chair at night instead of someone “assisting” her to bed. PLEASE SEE PHOTOS! When moving family member the bed linens were removed to discover a very soiled mattress and cover. It was evident that the soiling happened over and over and was not addressed. Anyone changing linens on this bed should have noticed the soiling. The soiling was so bad, it had permeated the water resistant cover and ruined the mattress. There were multiple washable bed pads and a large stock of disposable bed pads that could have been used to prevent this issue. The family member always had disposable underwear available and if staff had taken the time to prompt a potty break and or change of underwear, the soiling may have not been an issue. The window in the bedroom that would not close properly was still broken. (It was reported about a month ago and temperatures have been below freezing!). The Isakson family needs to be VERY ASHAMED. This is advertised as a premier facility and their rates are VERY HIGH!

RE: Park Springs’ “response”: Park Springs is attempting to brag about their COVID response to the son of a deceased member that contracted the virus at their facility and died from it. The same son that told them she needed to go to the hospital a week before they bothered to get her there. Negative stars. My mother was a resident at Park Springs. She couldn’t write this review because she is dead. Please do your homework before you decide to send your loved one to any independent or assisted living facility. Unfortunately, we did move our mother to assisted living at Park Springs. It is a decision that I regret. In August of 2021, my mother was infected with a breakthrough COVID infection. As soon as I learned of the infection, I asked Orville DeGuzman, their licensed Assisted Living Community Administrator, to please make sure my mother goes to the hospital if proper care cannot be rendered on-site. Mr. DeGuzman said that rescue and/or the hospital would not take her because she wasn’t sick enough. I would later discover, at least according to the hospital, this was untrue. Why would he say that? I also inquired about getting her on the therapeutic IVs that were, by then, available. Mr. DeGuzman responded: “IVs are beyond an Assisted Living so restorative cocktails are not available to us to administer and she is not ill enough to require treatment at the skilled nursing level of care.” My mother was an extremely, extremely high risk case. We were assured her vitals would be closely monitored. In seven days, her oxygen dropped to 80, EVEN AFTER A BREATHING TREATMENT. So, we asked for the records reflecting her vitals. We had to ask Orville about the absence of O2 readings for the 2-3 days before her level plummeted to 80. According to Mr. DeGuzman, they were not noted because they were normal. Hmm. I didn’t know vitals were not recorded when they were “good.” We were previously told our mother failed a MOCA test, and that was at least part of the basis for Park Springs wanting to move her to Cobblestone. When they finally released the test results, we discovered, contrary to their direct representation, that she PASSED the test. Though they never told us, they gave her a subsequent MOCA test, presumably to try to get her to fail one. They never told us about that test. Why? I don't know, but she did even better on the second one. I will also be seeking to find out why the date was changed on the second test. If Parks Springs denies this in their response, I will post both MOCA tests publicly and let people decide for themselves. Unfortunately, my mother died as a result of her breakthrough COVID infection. Mr. DeGuzman then advised: “… I do not play a direct CARE role with members.” That seems at odds with telling somebody that a resident isn’t sick enough to go to the hospital. It also seems at odds with Mr. DeGuzman recommending another higher, more expensive level of CARE. Mr. DeGuzman also seemingly has poor recordkeeping. When the records were finally provided, Mr. Deguzman recorded late entries in the records for a period of thirteen consecutive days. Interestingly, it was just twelve days before my mother's death that he supposedly did this. Based on these records, Mr. DeGuzman late recorded his notes for all of those days in the span of 27 minutes all on September 9, 2021. Again, if Cobblestone denies this, I will post them. I simply want people to know what they are and are not getting with this company. Park Springs has not returned her share of equity in over 2 YEARS. "Geez we just can't figure out why it hasn't sold." Maybe because people aren't getting what they paid for. Again, I don't know. In my family's experience, there was a top-down lack of meaningful communication from Park Springs. They don’t respond to emails. Kevin Isakson assured us he would look into it and call us. That was untrue. Nothing. Sent follow up emails. Nothing. Please, please do your homework. Confirm in writing anything you hear from Mr. Isakson, Mr. Helms, Mr. DeGuzman, or any of the sales staff.
