Everyone that works there cares about the residents and has their heart in what they do. Alexis Farmer Administrator is intelligent, caring and tries to make the residents, families and staff happy. Wanda J. Scott DON is knowledgeable, caring and devoted to staff and residents.I have seen her jump in cleaning, bathing, feeding, and answering call lights. No job is too small or big for her. When COVID hit she was right there with us. When it snows she spends the night. The nurse managers and CNA's are so dedicated. I wish I could name them all. Housekeeping keeps that place so clean. It has never smelled like a nursing home. The kitchen staff are amazing. Easther bakes treats for the staff and residents that are so delicious. Nino, Carlita Mrs. Barbara, Hope, Angie, Daryl. They are always smiling and helping the residents, not just cooking up good food, but going above and beyond. Jasmine and Chastity are the best social workers, organizing and setting up a smooth transition for our skilled residents to go home. They are always setting up things for the staff and LTC. Activities are amazing. Angie and Veronica are great with setting up appointments, even accompanying residents when families can't go. Billie and Sam are the Bomb. Tracey is one of the best HR, going above and beyond. Maria, receptionist is the first smile you see. Bill and Red keep everything going. Mary makes sure we are always fully staffed. Catherine Adams dietician is so good with the residents and keeps everyone maintaining and gaining. Robbie and Lora are the best MDS. Therapy is wonderful, getting residents back to baseline. I have been here for 22 years. If something happened to me, I want Sabrina to get me some skilled days there. I wouldn't want to be anywhere else.

Staff was very knowledgeable when it came to administrative and clinical items! The activities committee were also very friendly and engaged with residents! Sabrina Davis and Jasmine Brooks are two great employees I had the pleasure of meeting! Amazing staff!
Im not for writing bad reviews but this place has tried to pull a fast one. Yes we understand that companies got to get paid but they way they tried to handle this process is throwing red flags not on for me but for the case managers, the insurance company and Duke doctors. Her insurance covers the rehabilitation at 100 % but this place was trying to force a payment but come to find out nobody there had even called the insurance company. Feels like someone the in finance office is trying to run scams on people but have been advised to not pay anything because it’s covered at 💯. Spoke to the health insurance they said it sound like a scam because there is no out of pocket. Just trying to get my mom better and home this has stressed her out and caused a set back. Not sure what Sabrina has going on but she wants to put her in under long term when it’s only going to be a short term visit. Just because your a 4.5 rating doesn’t mean you can treat people any way
As a pastor, I have visited numerous nursing facilities, and this one is the best. The staff is very friendly and helpful, and the facility is also very clean and bright.
Due to fire, our loved one was placed in Clarksville Health and Rehab. I was finally able to visit on September 11th. First of all, when I pulled in, the facility was so big! Like, an assisted living facility. Upon entering, the staff was very nice; everyone I passed greeted me. When I finally found my loved one, he was sitting at the table of the dining hall, which huge all by itself! Because it was my first visit, I wanted to see his room and let me just say, it was so neat and clean. Very nice furniture and his clothes were neatly folded and hanged. I was very much satisfied with Clarksville and I'm happy that our loved one is there; not sure for how long, but I know until change comes, he's in good hands. The nursing staff was on point with the residents; I can't stop talking about how clean the facility was; it didn't "smell" like a nursing home......it was so beautiful and I can't wait to get back!
