I am heartbroken and deeply frustrated by the severe neglect my father experienced over the past two years at Sage Point/Green Acres Nursing Home. He was a long-term facility resident, for many years, but the past two years, we noticed the decline. The lack of basic care and attention to his needs was appalling and caused his health to deteriorate drastically. Malnutrition: Despite multiple requests to improve his dietary plan, my father’s weight dropped significantly over time. He became severely malnourished, weighing only 100 pounds at his worst. It was clear that he was not receiving proper nutrition or assistance during meals, even though we raised these concerns over and over again. There was never a structured plan put in place to address his malnutrition, and this neglect ultimately led to his rapid decline and hospitalization. Poor Dental Hygiene: During his hospitalization, it was discovered that my father had severe dental neglect. His teeth were black and unclean, with no evidence that the facility had been taking care of his oral hygiene. This lack of basic care is simply unacceptable, and it contributed to his overall decline. Bedsores: My father developed untreated bedsores and pressure ulcers, which were noticed during his hospital stay. The facility assured us on multiple occasions that he was being moved regularly and did not have any sores, but this was clearly not the case. His bedsores were extensive and painful, further evidence of the facility’s lack of attention and inadequate care. Inadequate Staffing: The facility’s staffing issues were evident throughout his time there. It was clear that there were not enough staff members to provide the level of care needed. Mealtime assistance was inconsistent, and despite my father’s declining condition, he did not receive the supervision or hands-on care required. Lack of Communication: Over the years my father was there, communication was abysmal. We constantly had to chase down updates on his condition, and the staff often failed to provide clear answers. When we requested meetings to discuss his care, including during his hospitalization in 2024, we were told no meeting would take place until he returned to the facility, despite his critical condition. Conclusion: The neglect my father endured at Sage Point/Green Acres Nursing Home was inexcusable. From malnutrition, poor hygiene, and bedsores to inadequate staffing and poor communication, it is clear this facility does not prioritize the well-being of its residents. We ultimately moved my father to another facility, but the damage done to his health was immense. I would not recommend this nursing home to anyone seeking compassionate or competent care for their loved ones.
