AVOID THIS PLACE AT ALL COSTS. A family member was admitted to CareOne Holmdel acute rehab, which we chose after reading all the Google Reviews (and visited) that seemed relatively positive, after his post-hospitalization for pneumonia and blood infection, which lasted for a month. I was fooled. I strongly advise against sending a loved one to CareOne Holmdel Rehabilitation Center. The quality of care here is shockingly poor, and the staff is neglectful, unresponsive, and ill-equipped to handle serious medical conditions. Despite assurances that proper treatment would be provided, it never happened. He was supposed to receive medication to help reduce fluid buildup, but it was completely ignored. Instead, staff carelessly administered additional fluids, worsening his condition rather than improving it. This blatant mismanagement led to extreme swelling throughout his body, which the facility failed to properly monitor or document (and yes, I requested all the medical documentation, which I then used to speak with the staff very directly about--which led them all fumbling over my questions and their mistakes in his care). The neglect was not just medical but also physical. He fell out of bed, suffered injuries, and was left on the floor for 45 minutes before receiving any assistance. To make matters worse, the facility never even informed the family of the incident. His call light was broken for three days, leaving him without a reliable way to ask for help, and only after multiple complaints was it finally addressed. I would receive cell phone calls from my family member asking for help because they couldn't get help in the facility. They operate with a light system, so when a patient needs helps and presses the call button, a light outside the door turns on. However, if no one is in the hall at that time--there is no other way to know that a patient needs help, which sounds unsafe to begin with. Requests for critical medical scans and weight tracking were ignored, despite clear signs that his condition was deteriorating. When concerns about fluid retention and kidney failure were raised, staff dismissed them—yet, just two days later, he ended up in the hospital in severe kidney failure, with fluid dangerously accumulating in his body and lungs. What should have been a rehabilitation stay instead turned into a life-threatening situation due to sheer incompetence. This facility failed in every possible way—ignoring medical needs, neglecting basic patient safety, and outright disregarding family concerns (and we had multiple meetings called to address these issues directly). Their actions (or lack thereof) caused severe harm, and had the situation continued, the outcome could have been fatal. My family member's stay here resulted in a second hospitalization that lasted two weeks. The terrible administrator at this place then had the audacity to call me to ask me if I wanted to bring my family member back there. During this call, I expressed all the terrible things that happened--and the fact that all these behaviors led to severe conditions and almost death of my family member. Her response as "I am sorry for the slight inconvenience that you had here." I just told her to never call us again, and that we would file a complaint with the NJ Department of Health and hung up. I changed where my family member went after this experience and it's been so much better than this place. Do not trust this place with your loved one’s care.

The entire staff, from the RNs, PTs, OTs, to the aides and housekeeping personnel, were all friendly, efficient, professional, caring and kind. Response, once the call bell was pressed, was prompt. RN Mary was an absolute gem, always warm & friendly, and anticipating my needs. Gemma, Nikki & Wilicia in therapy were marvelous - without them my recovery would have been slow. They got me motivated and moving! Janet made sure meals were always delivered hot and the room was immaculate. I only wanted sh I remembered everyone's name so I could post them here, too.Ann brought afternoon tea just because she was super nice. I have high praises for the staff and facility and would recommend Care One Holmdel to anyone planning a rehabilitation facility. Thank you all.
My wife Diane had a great experience at Careone in Holmdel over the last three weeks. The staff was very responsive, Nurses, aides, as well as the Doctors. To our pleasant surprise the food was good as well. She was at a another facility two months ago that had a 5 Star rating on Medicare.gov which could not hold a candle to Careone. Which I would have to rate a 10 Star compared to the previous facility.
The Care One team took great care of my grandfather when he was at the facility. Knowing that my grandfather was is such capable, compassionate hands brought my family great comfort and peace. The staff’s kindness and professionalism to my grandfather’s well being made a remarkable difference in his life and ours. The patience and respect the staff demonstrated everyday did not go unnoticed and my family deeply appreciated the thoughtful gestures that helped my grandfather feel valued and cared for.
The staff at this facility are all extremely nice and patient with the patients. My mum was admitted here for exactly 2 weeks and I visited every single day for 8am-8pm and I can assure you that this is a great facility for your loved one to be cared for . The patient therapist , occupational therapist , speech therapist , the nurses and aids are all a solid 10 on a scale of 1-10
