My grandfather stayed at Canyon Creek for a little over a week. He was discharged here due to CCPA being a preferred partner with Kaiser Permanente. The nursing team, activity team, dietary, and environmental team are absolutely "GOLD STANDARD". As an 84 year old Indian male "nana" actually enjoyed the food. The DON and Executive Director are actively engaged in the care of all their patients. On the day I came into see Nana he was actually completing a ROM activity with the activity director!!!!!! I just wanted to thank the team!!! Nana is home and doing much better, thank you for being apart of his journey.

When I came to this facility, the people were extremely friendly. I felt good being here because I was being attended to immediately. Staff here took care of me in a very nice and professional manner. I really would highly recommend this facility to whomever is in need of this type of care.
Today marks my 1 year here at Canyon Creek Post Acute. I am amazed and thankful for Canyon Creek. The team here is great ! The facility is clean, staff is very friendly, the managers are great and well verse of their work. The Administrator is very hands on.
This facility helped me recuperate fast. The rehab team are so wonderful. They got me going. I was admitted weak and have been here for almost two weeks now and I'm way better that I was. The recreation/activities room was great. I had a great time socializing and getting to meet other wonderful patients. Machu and Mary are sunshine of this place. Thank you for all the staff! I highly recommend this place!
I would like to say the staff was really nice and caring I know my uncle is a hard person to deal with I appreciate all the staff and nurses and activity personnel and the physical therapist taking care of him while he was there. You all are awesome. Thank you!!
