I am absolutely blown away by the incredible care and attention my mother has been getting at Beaver Healthcare and Rehab! My whole family was hesitant to send our Mother to a facility but we knew fairly quickly that she was in good hands! The staff went above and beyond to make her feel comfortable and at ease. From the nurses to the therapists to the activities women, everyone was friendly, knowledgeable, and compassionate. The facility itself is much cleaner and well-maintained vs other facilities I have seen in the past. The rooms are cozy and comfortable, with plenty of natural light and fresh air. They have a nice big dining room where we were able to have the whole family visit. But what truly sets this facility apart is the level of care and dedication shown by the staff. They genuinely care about each resident as an individual, taking the time to get to know each one and using that info to provide the best care they can. They celebrate the small victories, like when my mother was able to get herself in and out of bed. I was worried about leaving our mother but honestly it has become a second home to her. We are all just so grateful for the care she received and would highly recommend this facility to anyone in need of skilled nursing care.

The care to say the least, was substandard, to say the most, negligent and abusive. I was there for physical rehab, because I was malnourished and had lost partial use of my legs. On one occasion, I had rung the call bell, and when no one came, I went to the restroom alone. I was supposed to have assistance. I fell leaving the bathroom. I called out for help, and a nurse named Maria came in the room. She immediately began to berate me for having fallen. And said "get up, use your walker *you're not paralyzed, stand up". "I'm not a crane, to lift you....I have patients all up and down the hall....my back hurts , and I'm not going to lift you." I crawled to the bed and tried to lift myself up. I couldn't. She came over to the bed still yelling, and lifted me up by my pants and onto the bed. I needed nausea medication, because the abuse made me feel like I was going to vomit. Don't subject your loved ones to this place. Most of the staff are rude and uncaring. I filed a report with the administrator. Nothing was done as far as I know regarding the incident. The exception to the neglect/abuse, was the PT/OT department. They were outstanding, and caring. I can walk with a walker because of them. I will always be grateful to them for all the support and assistance they provided.
Just over all bad. Workers were slow and place was not very clean. My fathers room was dusty and dirty. The bathroom was not cleaned. There was nasty stuff in his bathroom. That was there from the last time I visited. These workers are supposed to be here to take care of patients, not treat them bad. And, the people.who clean need to do a better job and start checking bathrooms and garbage. Will NOT recommend this place for anyone who needs to fund a Healthcare/rehabilitation center.
This place was AWFUL. My family member was treated with extreme disrespect from the nursing staff. They would yell at her when she rang the call bell saying things like “ are you serious? Again?” They slammed her cabinets and made her feel incompetent. My aunt had a UTI and was unable to use her left arm and leg due to several fractured bones after a fall and is also blind in one eye, she was unable to walk and unable to get to the bedpan on her own. They also neglected to set up transportation for a post op appointment a week after surgery therefore she missed the appointment and when I called to inquire ( I was very nice FYI) the nurse screamed at me and never once apologized for the missed appointment. My family member left there confused, she had NO cognitive deficits when she went there at all. She left there very depressed and not herself. Please don’t send your worst enemy here. I wrote to the PA Dept of Heath to report this information. I am a nurse and was appalled to hear how they treat the patients there. I wish I knew the nurses names who were verbally abusive but I don’t unfortunately.
I would like to thank everyone at Beaver Healthcare and Rehabilitation Center for all you do for my Mom. I feel that you provide an engaging and caring community for my Mom to now call “home.” She has been a resident at the Center for over a year now and is thriving. Even though she may not participate in all of the activities, you offer a wide range for all to enjoy as they may want. My Mom “touts” the deliciousness of your meals and it shows in her much needed weight gain. The staff has been on top of my Mom’s medical care since day one and played a significant role in improving her quality of life. I cannot say enough about everyone that I encounter and/or interact with at the Center. They truly care about my Mom and her wellbeing and are always willing to assist me and answer my questions. The Center’s Director, Leonard Quimby, has been an exceptional leader and liaison especially given all of the pandemic protocols. He takes on whatever you ask of him. He is devoted to the Center and its residents and staff and strives to make improvements where they may be needed. Kudos to all!
