I had nothing but the best experiences at Avenue of Lyndhurst , the front desk Marie was such a great help for our arrival. My Mums liked the upscale decor and her living area which boast with plenty of LIGHT!! the facility was clean and laundry was available. Huge thanks you ... to the business offices Ms.Shyla, Mrs.Williams staff , Nurses: Ms.Lyndsey Ms.Sharon, Evening nurse Mr. Stan. (all nursing aides )and ***physical therapists. Sad to leave.

I strongly urge you to reconsider any plans to send your loved ones to this place. As a current personal care and comfort professional that works within the facility, I have first-hand experience that leads me to believe this facility truly does not prioritize the well-being of its residents. In my role as an advocate for those living there, I witness daily neglect, with residents often left sitting in soiled conditions for extended periods without assistance. The management company Progressive Quality Care is more focused on maximizing profits than ensuring quality care, which results in chronic understaffing. The limited staff available is often not adequately trained or equipped to handle the challenges presented by the residents’ needs. A harrowing example of this neglect occurred on Christmas Eve. I unfortunately witnessed a woman who was forcibly discharged from the building, which happens often, but she was left outside in the freezing cold without any support or place to go. It was only after a dedicated STNA and a nurse spotted her that they rushed out to bring her back inside. Tragically, this woman later succumbed to her exposure. As a patient advocate, I have received multiple complaints from residents about persistent issues such as the water supply being uncomfortably cold and many individuals not receiving clean linens regularly. A lady last week was found on the floor by her son, and a man with dementia was left to roam around and tried to exit the back doors of the building until a visitor stopped him because there was no staff around. These are basic needs that absolutely should be met in any care facility. I implore you to think carefully about your options—do not place your loved ones in this environment. Do not come here!!!!!
Do not be fooled by the appearance of this facility, and do not trust the care your loved one will receive here. They are severely understaffed, and the organization prioritizes profits over quality care. I placed my complete trust in this team to manage my mother's care, and I visited almost every day to offer support and assistance. On Tuesday, January 7, 2025, at 11:00 a.m., during one of my visits, my mother experienced a seizure right in front of me. She had never had seizures but there was medicine on her tray that hadn't been administered by the nurse and she took it. I immediately pressed the nurse's call button, but no one responded. I called out for help, yet not a single person came to assist us. In desperation, I ran down the hall from room 130, yelling for help, and still, no one came. It wasn't until I reached the front office that someone finally helped, but by that time, I had already left my mother lying on the floor, completely unattended. Later, I was told my mom was fortunate that I was there; otherwise, her life could have been at risk, yet no staff arrived to provide care! Consequently, my mother ended up in the hospital and now I am forced to look for another facility! This facility must undergo a complete overhaul and be put on hold until the necessary changes are implemented. No one should have to suffer through such a traumatic experience and lack-luster and abysmal care! ***In response to the owner-1/24/2025. You already know what happened! As I spoke to your Opps Director and the Director, Angela was there when this occurred. So don't post like you don't know!**
My husband and I went to visit his bed ridden aunt at The Avenue at Lyndhurst this morning. Upon entering her room she notified us that she was contagious again. In early September 2024 she he had somehow contracted Scabies. She is bed bound and there are only three family members that visit her so we can not understand how she contracted scabies. When treatment failed to eliminate the disease she was admitted into the hospital for treatment. Eventually she was declared no longer contagious. Later during our visit I did confirm with the nurse that she had again developed Scabies. We don’t understand how she contracted Scabies again! I noticed she smelled like she needed to be changed. Her bedside table was cluttered with the disposable carry out container from her breakfast, various papers and a used wash cloth. I used sanitizing wipes to begin throwing out old papers and cleaning her table. I used a wipe to pick up the wash cloth and put it in the hamper in her bathroom. At that point I discovered that the toilet contained a bowel movement that had not been flushed, which is probably why I thought she needed to be changed. As I shook loose the wash cloth to drape it over the edge of the hamper, I could see fecal matter smeared throughout the cloth. The washcloth used to wipe her bottom had been placed directly on her bedside table which is where she had just finished eating her breakfast. While we have had repeated conversations with the various Director(s) of Nursing and the Social Worker over the last two years about the cleanliness of her room, specifically on her bed table including used medical debris, bandages, lotions, powders, etc. on her table where she eats, this situation exceeds being unsanitary. I immediately tried to talk to the current Director of Nursing but she had not arrived to the facility yet. Then I notified two floor nurses of the situation, both of whom said they would try to find out which aid had been caring for her. We also reported this to the Administrator as we were leaving. She apologized and said she would find out who was responsible. Unfortunately, it’s the same response we keep getting, but nothing changes. Complaint filed with the state and on the search again for a suitable long term care facility.
As the owner of a small in-home non-medical healthcare company, I have the opportunity to visit various care facilities regularly. Currently, I’m working with a wonderful family who calls the assisted living section of Avenue of Lyndhurst home. Thankfully, the family has expressed no complaints, which is a testament to the quality of care provided. What truly stands out to me at Avenue of Lyndhurst is the responsiveness of the Administrator and staff. They are always readily available to address any concerns, and when issues arise, they tackle them swiftly. Having spent years working the floor in similar facilities, I fully understand the challenges of staff shortages that many places face. However, at Avenue of Lyndhurst, the commitment to resolving any problems immediately has left a positive impression on me. It’s reassuring to see such dedication in action!
