My experience with this facility has been incredibly disappointing. The staff is unprofessional and clearly not invested in the well-being of their patients. Concerns brought up whether about care, comfort, or basic needs are either dismissed or ignored entirely. It's deeply frustrating to feel like you're speaking into a void, especially when the matters at hand affect health and dignity. patients and their families deserve respect, communication, and compassion none of which were consistently present during the time my loved one was here.

Do not send your loved ones here. Once you get past the admissions staff, that are more like sales people, the communication is poor. They truly do the bare minimum and that's even putting it nicely. Sent my father here after a stroke, he was back in the hospital within 2 days after an incident where they had to "redirect" him no one was contacted to be made aware of this or that he was sent away in an ambulance. This incident was the beginning of the end and this "rehabilitation" center did the complete opposite. My poor dad was Neglected to the max. No one knew his chart or issues or how to properly deal with someone in his state though they were aware of the issues before he was admitted. After being sent to the hospital, not once but twice. One of the doctors at a local hospitals said that they would never send him or any patient back to that facility if they had the choice. That says something. Zero stars. Do not recommend.
my mother is currently in the care of this so-called medical facility. She has told me about many instances of neglect and lack of care and empathy. From waiting over an hour to be changed and to having water rationed to her and her roommate. This is unacceptable and please don’t send your loved ones to this place if you truly care about them. I will not quit on this issue, and will make sure people know about this place, and that there is some accountability held for those responsible for the mistreatment of my mother. if you read this, please spread the word.
IF YOU LOVE YOUR FAMILY MEMBER, DONT BRING THEM HERE!! They dont do anything they say they're going to do. No calls for care updates, they never respond to email or calls, no controlled access for security (they let anyone in, especially on weekends) wound care is negleted, admin does not fill out important insurance and charting correctly. Will say they accept VA upon entry and then deny it after admission. There's not enough text available to describe how ungodly awful this place really is. Just go inside and look around for a few minutes, you should know enough by just that. There are a couple nurses that are great, but just not enough to justify subjecting someone you love to this level of incompetence and lack of care. The facility is run down and is sub par standards for 'Skilled Nursing' Good luck and do your homework!!! 0 STARS!!
Avante at Jacksonville Beach is a wonderful community with nursing staff that truly cares about the residents. They have wonderful private, semi-private, and quad rooms to accommodate all residents roommate preferences! The therapy team is amazing and has many success stories of residents that left better than how they entered!
