Review of Avantara Lincoln Park My mother is currently a resident at Avantara Lincoln Park. When she first arrived, she was placed on the second floor, where the nursing staff was kind and helpful. The environment was more pleasant, and any odors were minimal. However, after her Medicare coverage ended and we were waiting for Medicaid approval, she was moved to the 4th floor—which I believe is designated for more long-term residents—and the quality of care has declined significantly. The level of care on this floor feels substandard. Residents are often left in bed all day, not dressed, and my mother has not had a shower since she was moved to the 4th floor over a month ago—despite state guidelines requiring at least two showers per week. The floor has a persistent unpleasant odor and feels overcrowded and chaotic. We’ve encountered several aides with poor attitudes, and the nursing staff often appears to be agency nurses who rotate frequently. There’s little continuity, and many seem to lack empathy, making it feel like a revolving door of unfamiliar faces. Communication has also been a major issue. I’ve made multiple attempts to reach the nursing director and other staff, but calls frequently go to voicemail—which is often full and doesn’t allow messages to be left. The only staff member I’ve consistently been able to reach is the social worker. It’s incredibly disheartening to see such a dramatic drop in care and attention once Medicare coverage ends. Families deserve better transparency, communication, and consistency in the quality of care—regardless of insurance status.

Good evening my brother is currently residing at Avantara, The service and nurses are so helpful and nice, And they keep my brother room nice and clean, I have nothing negative to say it's all positive vibes. Very good guest service. And my Brother Victor on the second floor is doing so well with his therapy.
Mia at the front desk is awesome! My father is a patient there and we needed to get power of attorney forms together so that I can manage his affairs. She quickly offered to print the forms for me. Very good customer service. Thanks!
Avatars Lincoln Park, Is a very friendly and caring place from the time you walk in the door! They are a one stop shop, with all hand s on deck! The nurse was making sure my sister was getting the care Jean Moore needed. I took this picture and she didn’t even realize it. She wanted to make sure Jean was comfortable and safe. I give Avantara Lincoln Park location five stars. I took a tour of the facility today because I will be bringing my mother here. Having a parent who is vulnerable and needs care at a facility can be a very scary endeavor. Arthur Mitchell guided me through the tour explained services And showed such empathy and confidence in the quality of care here that I will be bringing my mother from the front desk staff to admissions. This has been a wonderful experience. I highly recommend coming to visit this facility and having your loved one here.
