This place is horrible. The food is always chicken for lunch or ‘dinner’ as they call it. My grandma has been sitting in a dirty diaper for the past hr. We have been pressing the button and the only time they come in is to give food and not change her. This place has changed my grandma for the worst. Not talking to any one and having lots of sores on her body. And on her ankles from the pressure points because they don’t move her around. This place sucks. I would love to rate it a zero but I can’t

The care I received during my week stay in SubAcute Care was just what I needed to enable me to go home. The PT was outstanding. Kudos to Dawn and Olivia. Everyone was friendly, positive, encouraging and upbeat. If you ask a question and it wasn't in their area line of duty, they found help for you. They went the little extra mile and that is appreciated. I enjoyed meeting the ladies in the Dining Room near the end of my stay.
The caregivers at Autumn View are incredibly attentive and treated me like one of the family. The facility is clean and well-maintained, and the staff is always prompt to address any needs, making me feel confident I was in the best possible hands. I want to give a special Shout Out to the Physical Therapy Team and Nursing Staff for providing such excellent care during my stay. I am very thankful for the care I was provided at Autumn View and would highly recommend them.
My grandma Cheryl Altschaft is here the rooms are really calming. the beds are really comfy and the food is great. The nurses are really nice and take great care of the patients everyone is very nice and welcoming great energy. If I had to recommend any were to go to rehab it would be autumn view
My father Brian Scholl was a patient there in 2020. He got a bed sore on his lower back. Nobody noticed or cared . He was there for a few months. We didn't know either. All though now we understand why he smelled like rot. Then he went to Elderwood another hole. They didn't notice either until i asked why he smelled. He couldn't feel his legs so he was clueless. Finally the hole Elderwood looked and stage 4 bed sore where you could see his spine! He was supposed to have back surgery. We suffered for almost 4 years waiting to heal . He died May 16 2025 of sepsis !! These so called rehab nursing facilities are hell on earth !!!!!!! I will never forgive these so called health care providers and you shouldn't either.
