My husband was at Autumn Lake for about a month. He was kicked out for behavior issues such as verbal abuse, threatening, hitting walls and vandalism to name a few. While I understand them not wanting him there, it was their brilliant idea to send him home to a 57 year old disabled wife. Within 24 hours he was removed by police and ambulance for threatening me with bodily harm and trashing our house. So I would just like to thank them for their lack of support and recommend that if a patient has a traumatic brain injury and cannot be kept there for safety issues maybe help the family find a better solution. I will say the nurses there are absolute angels and tries their best. Also best of luck calling there to check on a loved one. Most of the time it just rings and rings.

It's a one because I couldn't select 0. My grandmother was here 2 years ago when it was still the Reservoir which was owned by Athena. The overall service has significantly declined. What could the contributing factors be? Well I'm going to go with facts. The facility is now under the management/ownership of Autumn Lake, whereas 2 years ago it was an Athena facility. Clearly Athena expected more and Autumn Lake has a much lower standard. This is evidenced by several facts. Fact one being that my grandmother was in the same grey sweatshirt from the day she was admitted until two days AFTER I was verbally and (very aggressively) I might add confronted by the Director of Nursing via a phone call. On the DAY after the director verbally confronted me on the phone I visited my grandmother and she had the same shirt on. I sought out the director and brought her down to my grandmother's room and it was changed the FOLLOWING day, which was her fourth day there. That is unacceptable. While waiting outside of my grandmother's room for her to receive care, myself, and 2 other relatives sat and watched as the charge nurse and 2 other employees walked in and out of another resident's room where there was an "isolation precaution" sign as well as an isolation cart outside of that resident's room with the proper PPE for staff and visitors to use, none of the staff did. I asked a member of the staff if that resident was on precautions and she said I don't think so, then she said no they aren't. I asked the supervisor the same on my way out and she confirmed that the resident was on isolation precautions. That is an absolute violation and a huge infection control issue that honestly should be reported to the state. The most obvious difference between the then Reservoir and the now Autumn Lake would be the management, as they are running the building. That would include the current Administrator and the Director of Nursing. I know for sure that they could do better because it was better. When my grandmother was there 2 years ago we had ZERO complaints. I hope that this review helps someone who is looking for a skilled nursing facility for a loved one prior to placement, if so my message to you would be please look elsewhere.
My mother was there for two weeks following surgery. Nurse staff was good and attentive, thought stretched thin. Operational communications was inconsistent. Alarmingly, my mother’s new heart monitor was never turned on or monitored for her entire stay! Staff was not notified of her discharge and food service for gluten free dairy free diet needs was inconsistent for her and another patient on her floor. We appreciate the location as West Hartford residents and this facility has improved since our last experience two years ago.
I love Autumn Lake of West Hartford.As my Mom’s healthcare needs have increased over the years, it has become more challenging for me being her advocate and living so far away. She was in another facility prior to Autumn lake that was failing her and I in so many ways. My mother’s God daughter recommended I look at Autumn Lake. I am so glad I did! What an amazing difference. Everyone and I mean everyone has been wonderful from the beginning with my initial contact with Admissions Director. I receive phone calls with updates , the nursing care is stellar, the Nurse practitioner is on top of her care and the Administrator is top notch! I always say great leadership trickles down to the rest of the staff. Great leader= great staff. I would be remiss if I didn’t mention the Activities dept. My mom is a social butterfly and participates in all the goings on there. Bingo and music entertainment are her fav’s. It’s hard to keep my mom on the phone long because she is usually on her way to another fun activity she can’t miss. My mom tells me often and I quote” I love it here!” You don’t know how much peace of mind that gives a daughter who just trying to do the best she can for her parents. Autumn Lake was the light at the end of a very dark tunnel for our family. Thank you Autumn Lake of West Hartford!!
Unfortunately, I witnessed the weekend nursing staff spending more time on THEIR CELL PHONE then responding to the call lights that are incessantly blinking! Their lack of empathy towards these fragile human beings is medically negligible and dangerously harmful! My experience dealing with these nurses to assist in just getting them to do or respond to the care of my own mother is unbelievably unprofessional and medically negligent. My recent experience was so disgusting! I had to go through a caseworker to have a medical list faxed to my mother’s doctor! It took a week and a half to accomplish this task!!! I lack faith in this facility and the nurses employed there. I feel that I MUST be extremely proactive in my mother’s care. These nurses do NOT CARE!!! Their CELL PHONE is more important than the care of these fragile human beings.
