I spent a few weeks at Autumn Lake for physical therapy. All the staff are excellent! They were kind, caring and always available in a courteous manner. The building is clean and fresh. Lee does a great job in housekeeping! Z, Colleen Cheyenne and Mary in nursing are really on top of their game! Ada and Michael kept me engaged with activities. The Administrative staff were attentive and accommodating. My recovery with the therapy staff was remarkable. I will be recommending Autumn in Lake to anyone who needs Rehab!

Very happy with the care my mother is receiving. Any questions I have are answered promptly. Everyone working there is very friendly and Bridgett from hospitality goes above and beyond to make sure you’re happy, satisfied, and have everything needed Update: during a recent cold snap, my mother’s room was very cold. Expectedly had to use extra blankets to keep her warm and whoever was visiting had to keep their jacket on. Also had a discussion with an (the) administrator. Was not treated respectfully, and was criticized for wanting to check with Medicaid attorneys, (being used for Medicaid application) before proceeding with a current situation. Again, nurses and aids were excellent, doing all they could for mom. Then my mother was sent to the hospital because what the attending physician at hospital called “an incorrect diagnosis”. My siblings and I decided it would be best to have our mother go to a different nursing home when it came time for her to be discharged
My father spent a month at Autumn Lakes in Old Bridge recovering from a pelvic fracture and it nearly killed him. He lost 25 pounds and developed 3 pressures wounds. My father is a diabteic and he is on dialysis, his transport was routine arranged late and he often did not receive adequate time on the machine. This led to him being readmitted to the hospital. This was a blessing because we were finally able to transfer him to different facility. While some of nurses and PTs at Autumn Lakes were very knoweldgeable and friendly, this facility is chronically understaffed. My father was not properly bathed for almost 2 weeks. And his PT was limited to moving him to a chair until we complained. The food had limited nutritional value and is generally inedible. You cannot feed people that are sick hot dogs and expect that they will get better! The facility is run down and dirty. If you love your family member DO NOT send them to Autumn Lakes!
I have been visiting a patient in rehab and I have been impressed with the cleanliness of the facility and there is no institutional smell. No cooking smells, either. The staff seems friendly although I haven't interacted except when I come through the main entrance. They have lots of activities for the residents. The outside grounds have beautiful flowers and plants. Inside there are always attractive arrangements on the tables in the dining area and attractive art work on the walls in the corridors. Bravo, Autumn Lake, keep up the good work.
The entire staff is really terrific , and I mean the entire. Nursing, activities, PT, aides, housekeeping, social worker, administration, secretary; all very helpful, client concerned, and all out pleasant to deal with. Very happy with the progress my family member has made in such a short time and I know he will be back to his baseline very soon . Highly recommend Autumn Lakes.
