The facility at Autumn Lake Healthcare at Glade Valley was clean and the OT/PT services were excellent, but the overall care fell far below expectations. I was assigned different agency nurses daily, resulting in inconsistent and, at times neglectful care. Upon arrival, I was left in a soiled brief for hours. Communication with my family was misleading, and the food quality was very poor. When my insurance coverage ended, I was discharged the same day with no at home nursing or therapy arranged. The lack of coordination and transparency was deeply concerning.

All of the staff has been very kind and competent. They have cared for my wife's needs also providing PT, OT, and speech therapy daily. We do not hesitate to recommend this facility. Irvin Stapf
I’ve been visiting the Glade Valley nursing home recently on several occasions and I have found their care to be very good. The staff seem responsive and kind and the facility seems very clean. My friend has commented positively on the food.
Very friendly- from the front desk, to the staff, to the residents. I was able to stick around a little during their Bingo game and they were having fun. It seems to be a really nice facility and the staff truly care.
My first time to pick up my brother in law, Joel Warthen. The staff were extremely helpful. Being a retired LE and Security Director I was impressed with security protocols with a “smile”. I was not on the “pick up” list. Staff explained my situation and helped me understand their responsibilities for Joel’s safety. They quickly made checks to expedite my authorization. They even thought to gain permission to allow me pick up privileges in the future. Very resident and customer friendly.
