I left a review two weeks ago and not seeing it. I am so heart broken right now. (Today I am really heart broken My Mom passed away on the 6th of May 2025.) My mom fell and broke her hip. We picked this rehab hoping they can get my mom to walk just a little bit. On Tuesday April first my husband and I sat down in a meeting. She was walking if you held her hand and going up steps. Found out the next day on the second she was going to be released on Sunday. The next day on the third she was not the same person could barley talk could no twalk at all her right thigh was red and very hot at the touch. Had to get the nurse several times I kept saying I think its infected they told me we are monitoring it she was probably over worked at therapy. She was put on oxygen. On Friday I was still very concerned about her right leg. I kept asking if it was normal they said its because of her enlarged heart. she has had an enlarged heart for a while now. They said because she is immobile but several years ago she had broken her ankle and had surgery She was immobile then but her leg was not leaking like that. So I kept asking are you sure this is okey. I was told if you want to take her to ER you can. I backed off (biggest regret in my life.) and said you are the medical pro. Long story short she came home on Tuesday at 2pm could not walk or talk just cried out for help. On Wednesday at 9am home health nurse took one look at her leg and said take her to the ER. Found out she had a huge infection it was even in her blood. Also found out her heart was her normal it did not get any larger. She was in the hospital for over a week and is still in bad condition. We put her in hospice (that is when I wrote the first review) All the nurses and doctors said that infection did not happen over night. She did not die from a broken hip. She was recovering. She died from an infection. (I will keep check to see if this review is still up. I want them to change their policies so this never happens to someone else.)

My mom has been there for awhile and loves the staff. Bernard is such a nice person and most of his staff are super sweet and care about their job deeply. The staff keeps us updated on her condition and needs and seems always pleasant.
My mother was a patient here for only a couple of weeks before she was rushed to the ER for cardiac arrest back in June. When I went back to get her stuff it's nowhere to be found. Her personal bag with her bible, debit card, ID cards and sentimental items among other items have yet to be recovered. The only thing I was able to receive was her phone and that was being held onto by a nurse that actually hand delivery it to me. My mother stuff have gone missing and the maintenance guy in charge told me he would call me when he find it. Still waiting on that call and its been 3 months. Someone need to be held accountable for stealing or misplacing patient items when they have leave for emergency.
Didn't properly discharge my dad, no nurse set up for his aftercare. Granted he's aware, can sign himself out but since day 1 he's been saying he wants to go home but the procedure should've been followed regardless of his desire to rush out. I his daughter is always called for anything else I should've been aware, could've explained to him wait 2 more days (rather I was ready to pick him up then or not, because this is new to me also so I was learning as I go) it was more of a get him gone vs using common sense, he would've stayed those 2 days to be PROPERLY discharged but of course if y'all let him go he's gonna go, who wouldn't. And his clothing always gets missing, long wait to tend to patients. Some staff are responsible & professional but of course family isn't there 24/7, not good for the ones who can't talk
Autumn care employees are only there for the money. They do not really care what condition your loved ones in. When I walked in to visit my mom, she was in a zombie state and they only called the ambulance once I said she is not acting like herself. Everyone there was acting nervous when I was doing their job for them that she needs an ambulance. Who knows what would have happened if I did not check on my mom in the so called care at Autumn care. Best thing for anyone to do is take your loved ones somewhere else instead of Autumn care.
