If there were 10 stars they would get all of them. Please see pictures attached. As a clinician in the area, I found the Eagle 1 facility to be above reproach in all areas. I was honored to work with them as my cousin was on hospice. He lived at the Eagle 1 hours for approximately 2 years and passed away Nov 17,2024 and the staff did not leave his side. The management and staff went above and beyond what was required of them. He was treated daily with kindness and respect and will say he was quite spoiled. In his last hours staff came in and said their goodbyes. These are precious memories. They were there on their off time and would come in just to spend time. The Administrator and Supervisor both came in the night he passed away. They are more like family now and would not hesitate to place someone in their care. Much appreciation to Dallas, Courtney, Candy, Britany and the staff at White Oak Hospice. You are all angels to me.

Worse place ever to put anyone in cognitive decline and can’t fend for themselves or ask for assistance. Serious communication issues with mngmnt and staff and provider. Had many falls which weren’t notified to family. Hostile roommate which made it impossible for our dad to use his room. He slept on the couch for 5 months and often found him laying on the floor. Often left in wet and dirty clothes. They keep residents medicated to deal with behaviors and set in front of the tv all the time. No activities as advertised. Always understaffed. Wounds not cared for. Food wasn’t good and no alternatives offered. No assistance with eating or drinking. Our dad was severely malnourished when we sent him home on hospice and died 10 days later. He suffered terribly from starvation. Lots of documentation. Feel free to contact.
Staff is great at caring for the residents. It's been difficult to be able to directly contact management for questions but the nurses and other staff have communicated very well and given great support.
Hit a rough patch with management but we are now on the right track! We are full of amazing, loving staff ready to care for all these great people
