Our mother was there unfortunately for 3 months. She was not properly cared for. There were numerous falls that we were not even called about. We found out while visiting from bruising. There were times we would visit and her breakfast tray was still in her room not touched, even though it was lunchtime. Her living conditions were very unsanitary. After the last fall there, I told them I wanted her to go to the hospital. I told them I would put her in my car and bring her, if they did not call an ambulance. At the hospital, she was very dehydrated, had lost most of her kidney functions and had a blood bacterial infection (from the unsanitary conditions) she lived in. In short, mom entered walking with the aid of a walker to being bed ridden and on the verge of death after only 3 months of her time there. Needless to say, after 10 days in the hospital, touch and go, mom did not return to Ascension Oaks. We found a real place that cared for her exceptionally. I would never recommend Ascension Oaks to anyone who cares about their elderly loved ones.

I was here for 3 and 1/2 months from December 2018 through March 16, 2019 for rehabilitation due to a broken hip. The staff including the administrator Brett and the social worker was so good to me while I stayed there. I would highly Recommend Ascension Oaks nursing home and rehabilitation.
Horrible treatment to these patients. They deserve to be treated with dignity and humanity not neglected and ignored. The place needs to be cleaned out and straightened out. They have become complacent. Their staff needs training and the place is not clean. Upper management needs to go. Do Not leave your family here until things change.
Neglectful...lack of supervised staff...jail food...mistreatment moving residents by grabbing back of pants giving wedgie...uncaring CNA's sitting around on phone not helping residents...broken equipment...dirty rooms...traumatizing showers...lack of trained memory care staff
The staff is very messy they don't take care of the patients well they worry more about mess and what's going on outside the facility when I go their I pass through the back half of the staff is sitting outside smoking and on their phones when I used to call in the day time who ever the representative for the facility is very rude and wants to know every detail instead of letting me talk to somebody over her the facility is not clean the staff needs training because patience is dying often at the facility I wouldn't recommend anyone to go here in Gonzales Louisiana
