There are unqualified LPN and CNA the have no respect or compation for the residents.my mother's personal belongings were stolen and never returned . When my mother passed there was not an RN on duty in the whole facility. After several text messages attempts to disguess the situation with the administration I had no responses. Please do not place my your loved one in this facility.

My mom spent the last few weeks of her life here and was well taken care of. The staff was attentive and listened to our concerns and made sure mom was comfortable until the very end.
Apparently under new management, expect a zero to one star experience here at best. Came here for short term rehab after knee replacement. I’ll start with the positive, PT staff is excellent. Beyond that, what a wretched experience. Nurses are abysmal, would not listen to me and would go as far as to be outright mean to me. I understand that their job is extremely difficult and they seem severely understaffed but i did not experience an ounce of empathy. I would ring the bell for assistance and no one would ever come, only to hear them talking in the hallways. No one would ever help me to the bathroom at night when i was feeling very unsteady on my feet. I was often feeling hopeless, unheard, and even vilified by them. They are careless and unprofessional. Food is edible but of the absolute lowest quality. Cold deli meat sandwiches on white bread every night for dinner. My “care team” was habitually behind on my status, seemingly they have the bare minimum of communication between staff… obviously management is the problem; it trickles down from the top and only gets worse the further down you go. I would caution any one with an elderly family member looking for long term nursing care against placing their family here. You can rest assured they will get the worst possible care, will be gaslight and made to feel like they’re the grandma from Happy Gilmore. Zero stars would be a more accurate score.
My Mom is a resident at Country and we are very grateful for the excellent care she receives from the hardworking nursing team. The staff in all departments are caring and responsive. The High Tea on Mothers Day was lovely, a big thank you to the Activities department!
Country Center has been the perfect place for rehabilitation for my loved one. This facility is a safe, caring nursing home where everyone is well cared for. The entire staff are all kind, thoughtful, attentive, and always a pleasure to talk to! They go above and beyond to ensure that patients are well taken care of everyday!
